Based on that inspection, the enclosed punch list of maintenance/defect (M/D) deficiencies was prepared. Within fifteen (15) working days of the date of this letter, please contact the inspector, Terry Smith, at 206-296-6560 and Chris Treichel at 206-296-3308 with the schedule for completing these items and indicating when work will begin. When all items are completed, a follow-up inspection will be held. When we determine that all deficiencies have been corrected, we will notify the appropriate agency of our approval for release of the appropriate bond. ROAD SURFACE AND IMPROVEMENTS: Roads: I) Seal the joint in the southerly (west bound) lane (equal to the length of the project). 2) Seal the perimeter of the (ACP) patch at the east end of the project, west bound lane. The cracks, deterioration or both in the sidewalk panels at the following locations will require the damaged sections to be replaced (5-foot minimum). Install expansion joint (typ.) between the placement of the new and existing sidewalk. Sidewalks: I) Sweep up debris at back of the walk/face of wall at the west end of the project. 2) Sweep and remove gravel and dirt from sidewalk surface, south of fire hydrant, and west of retaining wall. 3) Crack in sidewalk panel, 17 feet west of the east end of wall, north side of road. 4) Extend ACP to R/W at driveway 12122 (to prevent gravel from encroaching on sidewalk). 5) Remove and replace cracked sidewalk and curb and gutter at driveway entrance, westerly panel (6 lineal feet minimum). 6) Cut back sticker branches to back of walk, located easterly of driveway entrance. 7) Sweep off sidewalk surface, STA. 50+63 (Prudential for sale property). 8) Remove vegetation growing between back of curb and face of sidewalk on south side of the road. Any extensions to the 30-day limit must be approved by the engineer. Repair and backfill the rock "retaining" wall on the east and south side of the vault. 2. Cut the 12-inch cleanout risers flush with the top of the vault, and grout the cleanout frames to the vault. 3. Ins.tall Yz-inch stainless steel Allen head locking bolts on the access riser to the vault. 4. Drain the vault and remove all sediment accumulation and dispose of in accordance with applicable regulations. 5. Water levels were well below the invert elevation in the vault. When the vault is drained, grout all cracks and seams to ensure the vault is watertight. He'll be onsite tomorrow (Friday, 2/20/09) to re-seal the joint in the west bound lane over the length of the project. His traffic crew will be taking a lane which will be in compliance with the contractor's original traffic control plan utilized during the initial project construction according to Dave A .. They will be starting up @ 9am according to Dave. This should work out good in order to avoid impacting rush hour traffic. I also notified Dave of a concrete panel in which they removed and replaced with crushed rock toward the west end of the project, adjacent to the wall. I notified him that panel is required to be replaced in kind to the surrounding area (concrete). He agreed and said it was just a oversight. Will you please fill in for me if you can. He would like to go over the p-list to assure all of the work has been completed to KC's standard.
