Temporary Use Permit approval. No appeals were filed, therefore, this decision is final.
Please note and adhere to all conditions of approval outlined on the Administrative
Temporary Use Permit dated August 30, 2013, specifically, condition #1 "the site shall
be left free of debris, litter, or other evidence of the temporary use upon completion of
removal. of the use." Your permit expires August 30, 2018, a final inspection will be
completed to verify all conditions have been met.

DESCRIPTION: The applicant is requesting a Temporary Use Permit for the
continuation of soil remediation originally undertaken under File
Numbers: LUA98-136, LUA00-135, LUA03-048, LUA06-010 and
LUA08-053. The continued operation is requested, until the
applicant could permit the development of a site closure
agreement. The project proposal is for two separate locations
both within the Residential 4 (R-4) zone. Each site contains above
ground treatment equipment within a 400 sq. ft. fenced enclosure.
All development currently exists on site. The project is located at
3524 SE 5th Street, Mapelwood Park and 3400 SE 6th Street.
The Temporary Use Permit is hereby Approved with Conditions* and subject to the following
conditions.
*CONDITIONS OF APPROVAL:
1. The site shall be left free of debris, litter, or other evidence of the temporary use upon
completion of removal of the use.

RECONSIDERATION: Within 14 days of the decision date, any party may request that the
decision be reopened by the approval body. The approval body may modify his decision if
material evidence not redily discoverable prior to the original decision is found or if he finds
there was misrepresentation or fact, After review of the Reconsideration request, if the
approval body finds sufficient evidence to amend the original decision, there will be no
furtherextension of the appeal period. Any person wishing to take further action must file a
formal appeal within the 14-day appeal time frame.
APPEALS: Appeals of the per mt issuance must be filed in writing on or before 5:00 p.m. on
September 13, 2013. Appeals must be filed in writing together with the required fee with
Hearing Examiner c/o City Clerk, City of Renton, 1055 S Grady Wy, Renton, WA, 98057.
Appeals to the Examiner are governed by City of Renton Municipal Code Section 4-8-110.
Additional information regarding the appeal process may be obtained from the Renton City
Clerk's Office, 425-430-6510.
EXPIRATION: A Temporary Use Permit is valid for up to one year from the effective date of
the permit, unless the Community & Economic Development Administrator or designee
establishes a shorter time frame or an applicant can request that a permit be valid beyond
the one year expiration, for up to five years at the time of application or prior to permit
expiration.
EXTENSIONS: Extension requests do not require additional fees and shall be requested in
writing prior to permit expiration to the Department of Community & Economic Development
Administrator.

OPLC Maplewood Block Valve Temporary Use Permit (TUP) Tier 2 Application for the OPLC block valve site located
at SE 5th Street and Newport Avenue SE in Renton, Washington. The TUP is being submitted to renew the existing
TUP LUA-08-053 expiring on June 13, 2013. Antea Group is requesting continued use of the remediation system at
the site at least through 2015 to complete site closure investigations in agreement with the Washington State
Department of Ecology Model Toxics Control Act. Once the cleanup is complete and a No Further Action
determination has been issued, the systems will be decommissioned and the equipment enclosures and their
contents will be removed.

A Temporary Use Permit is being requested to maintain the use of the existfh/f s.@l!i),,and north
equipment enclosures in the southeastern portion of the Maplewood Park, and just north of Maplewood
Park across SE 5th Street next to the Olympic Pipe Line Company (OPLC) block valve enclosure. Both
remediation equipment enclosures are located in an R8 zoned area. In November 2012 Antea Group
requested a Demolition permit from the City of Renton to permanently remove both equipment
enclosures from the site. It was determined a No Further Action letter from the Washington State
Department of Ecology (Ecology) would be required prior to system removal. The Temporary Use Permit
LUA-08-053-TP was extended on December 3, 2012. Following correspondence with Ecology, Antea Group
was unable to attain a No Further Action determination before the permit expiration in June 2013. At this
time Antea Group is requesting continued occupation of the remediation system until a No Further Action
determination is obtained from Ecology.
In early October 1986, a petroleum release was discovered in the vicinity of an OPLC block valve and
pipeline ROW in the Maplewood neighborhood of Renton, King County, Washington. An estimated
80,000 gallons of mixed gasoline, diesel and jet fuel were released. Site characterization, remediation,
and monitoring activities were conducted at the site between 1987 and 1988. Groundwater monitoring
continued until 1993 and was re-initiated in 1997. In March 1998, the site was entered into the Ecology
Voluntary Cleanup Program (VCP).
Active remediation was re-initiated in December 1998 and continued through November 2005. A
remediation pilot test was conducted in early 2006; however, in May 2006, all active remedial activities
were discontinued. Remediation equipment at the site is no longer operational. The primary system was
shutdown based on the conclusion that operations at the site have reached a practical end point, and that
the remaining concentrations in groundwater were not decreasing in response to operations but instead
to natural attenuation. There is no plan to restart the system.
Currently, quarterly groundwater monitoring and sampling has concluded at the Site. Groundwater
analytical results indicate that concentrations of dissolved-phase hydrocarbons have been below the
respective Model Toxic Control Act (MTCA) Method A Cleanup Levels for four consecutive quarters in all
groundwater monitoring wells associated with the Site. On December 26, 2012, Antea Group sent
Ecology a Remedial Investigation Report regarding the independent cleanup of the Site. Ecology
responded in a letter dated March 5, 2013, which stated that further action was necessary. Additional
assessment of subsurface soil is required to obtain a no further action determination from Ecology.
The physical components of the remediation system include two equipment enclosures constructed in
November 1998. The enclosures are approximately 400 square feet each and contain the aboveground
treatment equipment. The northern equipment enclosure is situated adjacent to the west side of the
existing OPLC block valve enclosure. The southern equipment enclosure is situated west of the existing

playground in Maplewood Park. The equipment enclosures consist of a concrete pad surrounded by an 8-
foot high chain-link fence with sight-obscuring slats.
The air sparging system consists of a series of 2-inch diameter vertical air sparging wells located within the
block valve property (at 3524 Southeast 5th Street), the north and west sides of Maplewood Park (3400
Southeast 6th Street) and next to the eastern curb of Newport Avenue southeast (between 3330 Newport
Avenue Southeast and 3400 Southeast 5th Street). The air sparging wells are about 25 feet deep. The well
monuments are flush with the ground surface, or covered with sod.
The vapor extraction wells are connected via shallow underground piping (2 to 4 feet below the ground
surface) to air compressors situated in the above ground treatment equipment enclosures. The
underground piping is located beneath the following area: (1) the southern margin of the block valve
property, (2) Southeast 5th Street between the block valve property and Maplewood Park, (3) the north
and west sides of Maplewood Park and (4) the grassed planter strip on the east side of Newport Avenue
Southeast (between 3330 Newport Avenue Southeast and 3400 Southeast 5th Street).
Antea Group is currently working with Ecology to develop a confirmatory soil assessment work plan to
determine current subsurface conditions. This next phase of operations, through the end of 2014, is
expected to bring the site to closure. Once the cleanup is complete and a No Further Action
determination issued, the system will be decommissioned and the equipment enclosures and their
contents will be removed.
