The mechanical permit was acquired through an on-line permit system MyBuildingPermit.com. The compliance date was April 3, 2018. (Exhibit#2) March 19, 2018: A Warning of Violation was sent to Mr. Ho requiring him to provide additional information for mechanical permit. The Warning was sent USPS Tracking Mail #9114 90149645 1403 0107 41 and was delivered to Mr. Ho's mail box on March 21, 2018. The compliance date was April 3, 2018. (Exhibit#3) April 24, 2018: A check of permit records indicated the additional information had not been received by the building department. A second Warning of Violation was issued to Mr. Ho requiring him to provide additional information for the mechanical permit. The second Warning was sent USPS Certified Mail #7016 1370 0002 0771 2082 and was returned to the city on May 22, 2018. The compliance date was May 9, 2018. (Exhibit#4) May 22, 2018: A check of permit records indicated the additional information had not been received by the building department. A civil Finding of Violation was issued to Mr. Ho in the amount of$250.00. The Finding of Violation was sent USPS Certified Mail #7016 1370 0002 0771 2266 and USPS Tracking Mail #9114 9014 9645 1403 0112 81. The certified mail was signed for on May 29, 2018, and the tracking mail was delivered to Agent on May 25, 2018. The grease duct for the type I hood has also been re-located which is going to require some assembly concerns to satisfy section 507 of the IMC. fN Please provide drawings to satisfy the minimum assembly requirements for his mechanical work. Commonly the arr existing buildings an approved fire wrap is installed around this duct. This fire wrap will require a special inspection to verify that its installation meets the manufacturer's requirements. Incorrectly installed or maintained grease ducts has caused many building fire. This is such a crucial building feature that we have placed the above permit on hold until the required design has been submitted reviewed and approved. Code Text: Roof Top Exhaust Relocation Corrective Action: The grease duct for the type I hood has also been re-located which is going to require some assembly concerns to satisfy section 507 of the Intemational Mechanical Code. Please provide drawings to satisfy the minimum assembly requirements for his mechanical work. Commonly in existing buildings an approved fire wrap is installed around this duct. This fire wrap will require a special inspection to verify that its installation meets the manufacturer's requirements. Incorrectly installed or maintained grease ducts has caused many building fire. This is such a crucial building feature that we have placed the above permit on hold until the required design has been submitted reviewed and approved. A check of permit records on May 22,2018,indicates we have not received the additional information requested to process,inspect and final the mechanical permit.This hood is in violation of city code.
