SCHEDULE B—Extension/Request for Additional Services
Relationship To The Original Contract and Purpose: This is an addendum to the existing WebOA Services Purchase Aereement
executed between WebQA,Inc.("WEBQA")and the city of Renton,WA("Customer")signed 12/16/2016 with its current term
starting 12/15/2017 and ending 12/14/2018("Current Contract Period"). The purpose of this Addendum is to add the new services
selected below.
Additional Services:Customer is orderin4the following services at the following prices.
Subscribed Services
GovQA State of WA Public Provides specific State of WA reporting module to comply with House Bill 1594 reporting requirements.
Records Reporting Module Include associated means for tracking and reporting to include:
• Metric 2:Average Time to First Response
• Metric 3:%Requests Closed Within 5 Days
• Metric 3:%Requests with Estimate>5 Days
• Metric 4:Estimated Average Time to Close
• Metric 5:%Requests with Estimates Sent
• Metric 5:Actual Time to Close
• Metric 6:Requests Seeking additional Clarification
• Metric 7:Requests Denied with Reason Count
• Metric 8:Requests Abandoned
• Metric 9:Total Request by Type of Requestor
• Metric 10:%Requests Fulfilled Electronically
• Metric 10:%Requests Fulfilled by both Electronic and Physical Records
• Metric 11:Requests requiring Scanning of Physical Records
• Metric 12:Estimated Agency Staff Time Spent on Request
• Metric 13:Total Staff Time Costs Incurred
• Metric 13:Average Cost of Staff Time per Request
• Metric 17:Invoice Payments Received(Must Use GovQA Invoicing)

Start Date and Timeframes: The START DATE for the term of this agreement will be 6/15/2018 and go to a SYNC DATE of
12/15/2018 at which point the term for these Additional Services will sync to the term of the Original Contract and all subsequent
renewals. The rest of 2018 will be prorated for 6 months at$750 and then Sync to existing contract
Billing: Billing will occur on the START DATE of 5/15/2018 and include the annual fees and implementation fee associated with
the Additional Services described above. After the SYNC DATE,the billing period for these Additional Services will be incorporated
into the billing of other WebQA services to the customer consistent with the Original Contract and all subsequent renewals.
BY SIGNING BELOW,CUSTOMER AGREES TO THE TERMS AND CONDITIONS OF THIS ADDENDUM.
