Hi Angela,
Thank you very much for your call back on Friday afternoon. I found the Modification Request Form on
the City’s website and have filled it out for the addition and alterations project for the Hattatolgu home
at 1211 N 32nd St. The completed Request is attached. Please let me know if I should also send the
drawings. A photo of the existing house is pasted below; the entry porch is on the left side of the house,
out of view..
I would appreciate it if you would send an invoice; the homeowners have agreed to pay it.
