Duvall Avenue NE $711,949.52(this supplement increased by$10,541.52)
Description of Work
Engineering services for design improvements on Duvall Avenue NE to restore the paved surface,and fill in missing non-
motorized facilities such as sidewalks and bicycle lanes,illumination,and channelization.
As outlined in attached Exhibit A-1,this Supplemental Agreement Number 5 will include survey of east side of Duvall Avenue
between Northeast 7th Place and Northeast 10th Street,between the outside shoulder stripe of right-of-way.
The Local Agency of City of Renton
desires to supplement the agreement entered in to with Parametrix.Inc.
and executed on 06/21/2016 and identified as Agreement No. CAG-16-063
All provisions in the basic agreement remain in effect except as expressly modified by this supplement.
The changes to the agreement are described as follows:

Section 1, SCOPE OF WORK, is hereby changed to read:
The Scope of Work described in Section II of the Agreement is amended to add the Scope of Services described in the attached
Exhibit A-1.
II
Section IV,TIME FOR BEGINNING AND COMPLETION, is amended to change the number of calendar days
for completion of the work to read: No Change.
III
Section V, PAYMENT, shall be amended as follows:
Additional services added by this Supplemental Agreement Number 5 increase the maximum amount payable by$10,541.52
making a new maximum amount payable of$711,949.52.See attached Exhibits"A"through A-2.
as set forth in the attached Exhibit A, and by this reference made a part of this supplement.
If you concur with this supplement and agree to the changes as stated above, please sign in the Appropriate
spaces below and return to this office for final action.

Scope of Work— Supplement No. 5
Task 2—Survey and Basemapping (NTP through four months post-NTP)
Goal(s)
Work will include survey of the east side of Duvall Avenue between Northeast 7th Place and
Northeast 10th Street, between the outside shoulder stripe to of right of way. Topographic survey
will fill gap in existing topography, and confirm accuracy for existing features. Flowline of the
existing gutter on the west side of Duvall Avenue between Northeast 7th Place and Northeast 10th
Street will be surveyed to verify roadway width.
Assumption(s)
• Survey includes above ground features including edge of pavement, ditches, tops, toes,
signs, edge of vegetation, trees, etc. sufficient to produce a 1-foot contour interval map.
• Underground utilities will be marked by Applied Professional Services (APS).
• Storm and sanitary sewer lines as measured from structure measure-downs.
• Potholing is not included.
Deliverable(s)
• Deliverable will include an updated Autodesk C3D base file at 1"=20' scale mapping
(unless otherwise specified), and will include all information listed above.
