Itistheresponsibilityofallsiteworkerstocontinuouslyevaluatethesafetyofsiteworkpracticesandengineeringcontrols,andimmediatelynotifytheProjectManager,ProjectSuperintendentorForemanofanyunsafeconditions,hazardsorpolicyviolations.Thisplanwillberevisedorupdatedasnecessary.Compliancewiththisplanandallfederal.stateand‘localregulationsisrequiredofallemployees,subcontractorsandvisitors.WASTECHARACTERISTICSOFSITE:Excavation,removal,loading,transportationofallreasonablyaccessiblesoilsthatcontaintotalpetroleumhydrocarbons(TPH),volatileorganiccompounds(VOC5),and/ormetalsatlevelsthatexceedMICAMethodAIndustrialCleanUpLevels.Theexcavation,removal,andoff-sitedisposalofscrapmetaldebrisdiscovered/encounteredduringtheon-siteexcavationactivities.Importing,placementandmechanicalcompactionofstructuralbackfillmaterialsthroughoutthesite.Thedewateringofon-siteexcavationsandmanagementofstormwatersonthesiteasperpermitandregulatoryrequirements.Thedewateringoperationswillrequirethehandlingandoperationofpumpsandtheirassociatedhoses.Theremovalofselectedgrassesfromthewetlandarea.Theplantingofplantsandseedlingsintheon-sitebufferareaandtheoff-sitewetlandarea.Theperiodicmaintenanceandinspectionofthebufferandwetlandareasbyqualifiedpersonnel.Potentialchemicalhazards:VolatileOrganicCompoundsThereispresenceofVolatileOrganicCompoundsinthesubsurfacewaters,andpotentiallyinthesurface/subsurfacesoilspresentatselectedon-sitelocationshasbeenconfirmed.MotorOils/LubricantsThereisthepresenceoflubricating/motoroilshasbeenconfirmedinthesurfaceandshallowsubsurfacesoilsaswellastheshallowgroundwateratselectedlocationsonthesite.Gasoline(allgrades)Thereisthepresenceofgasolinehasbeenconfirmedinthesurfaceandshallowsubsurfacesoilsaswellastheshallowgroundwateratselectedlocationsonthesite.LeadThereisthepresenceofleadconfirmedonthesurfaceandareashavebeenidentifiedatselectedlocationsonthesiteChromiumThereisthepresenceofchromiumconfirmedonthesurfaceandareashasbeenidentifiedatselectedlocationsonthesiteThismaynotbeanexhaustivelistingofchemicalsfoundon-site,itisimportantifyouhaveanyquestionsorconcerns,tocontactyoursupervisorimmediately!!!!-EMERGENCYINFORMATION:Emergencyinformationwillbepostedinaconspicuouslocation.Ifaninjuryoccurs,takethefollowingactions;
2.Getmedicalattentionandemergencytransportationfortheinjuredpersonimmediately3.Notifyon-siteProjectManager,ProjectSuperintendentand/foremanpromptlySiteSpecificWorkplan:Asite-specificworkplanprovidesadministrativecontrolsthatmustbeinplacetopreventworkerexposuretohazardsthataworkermayencounter.Thehazardsandcontrolsdiscussedmaynotencompassalltheactivitiesbutisdesignedtoalertsupervision,workersofHHJ,sub-contractors,sub-contractoremployeesandanyvisitortopossibleprojecthazardsonthisproject.Thisplanalsodiscussesthepossibleactionstotakewhenonthisproject.AreviewofthesitesJobHazardAnalysisshallbefoundinthejob-trailer/officeandmustbereviewedandunderstoodbeforeyouperformanytaskontheproject.Therewillbeexcavation,drillingandremovalbyheavyequipmenttakingplaceonthisproject.AnyexcavationmustbelocatedbycallingtheUtilityNotificationCenterand/orthelocalutilitytolocateanyundergroundutilities.Power,water,sewer,telecommunicationsorgaslinescouldbepresentandmustbelocatedpriortoanyexcavation.Hazardssuchasfire,Vexplosion,electrocution,disruptionofserviceordischargeofwastecouldbetheconsequenceofnotproperlynotifyingtheUtilityNotificationCenter.Locatedutilitieswillhaveitsdesignated.coloringandmarkingmustbemaintained.IIunsure,re-callforlocates.Allexcavationsmustbeinspectedbyacompetentpersonregardlessofdepth,whoisfamiliarwiththeexcavationstandard,soilclassificationsandprotectivemeasuresthatmustbetakensuchasshoring,shielding,slopingbenchingorengineering.Neveraccessanunprotectedexcavation.Overheadhazardstopowerlinesandtelecommunicationmayexist.Knowwhattheoverheadlinesvoltagesare.Don’tguess,calltheutilitytoknowthevoltagesandtoverifythatlinesareeitherde-energizedorprovidewarninglinesiflinescannotbede-energized.TherewillbeareaswhichwillofflimitstounauthorizedpersonalespeciallyifthespecificareacontainshazardousmaterialatoraboveabovetheMTCAlevels.Theseareasshallbedelineatedbyacombinationofchainlinksecurityfencing,plasticsecurityfencingand/orhi-visdelineatortrafficdeviceswiththeappropriatevisibilityropeandortape.Theseareaswillbeclearlydefinedandcommunicated,andonlyauthorizedemployeesareallowedtoentertheseareas.Iftheboundaries,signagebecomeinneedofrepair,contacttheprojectmanager,projectsuperintendentorforemantotakecorrectiveactionimmediately.Duringthemovementofequipment,itisimportanttogettheattentionoftheoperator.Assumetheoperatoroftheequipmentorvehiclescannotseeyou.BeawareofyoursurroundingsatalltimesandweartherequiredPersonalProtectiveEquipment(PPE)A)!personnelon-sitemusthavetheminimalPersonalProtectiveEquipment(PPE)•Hi-VisibilitygarmentsthatareANSIClass2U;...—iL--.
