Dear Mr. Norcross,
This office has reviewed your request to extend an approved Temporary Use Permit (File
No. LUA08-053, TP).
The decision for the Maplewood Fuel Remediation Temporary Use Permit was issued on
June 13, 2008. The Temporary Use Permit for the continuation of soil remediation was
originally undertaken under File Numbers: LUA98-136, LUA00-135, LUA03-048 and
LUA06-010. This is a continuation of a 1998 project by the Olympic Pipe Line Company
(OPLC) to remediate a historic fuel spill leak in the Maplewood neighborhood. The
expiration date for the last TUP was June 13, 2010. The Maplewood Fuel Remediation.
TUP is granted a 3-year extension. The project expiration date has been extended to
June 13, 2013. This extension beyond the two years originally permitted is based upon
an Administrative Interpretation (enclosed) that provides the ability to extend TUP
beyond the two year time frame. This application is eligible for one more two year
extension before a new application would be required.

On behalf of OPLC, Antea Group is formally requesting an extension to Temporary Use Permit 12-037. The permit is
associated with two remediation system compounds located at 3524 SE SIh Street (Maplewood Park), and 3400 SE 6th
Street. The permit expired prior to the Washington State Department of Ecology (Ecology) issuing a No Further Action
determination for the site. The 4Q 2012 groundwater sampling data indicates that four quarters of groundwater data
below Ecology Model Toxic Control Act (MICA) Method A Cleanup Levels has been obtained. This is the minimum
requirement for a No Further Action from Ecology. Currently, Antea Group is working with Ecology to determine the
next steps for a No Further Action determination. Antea believes a No Further Action determination will be issued in
2013.

Project Description: The applicant is requesting a Temporary Use Permit for the continuation
of soil remediation originally undertaken under File Numbers: LUA98-136, LUA00-135,
LUA03-048 and LUA06-010. This is a continuation of a 1998 project by the Olympic Pipe Line
Company (OPLC) to remediate a historic fuel spill leak in the Maplewood neighborhood.
The project includes two equipment enclosures constructed in November of 1998 approximately
400 square feet each. The northern equipment enclosure is situated adjacent to the west side of
the existing OPLC block valve enclosure. The southern equipment enclosure is situated west of
the existing playground in Maplewood Park. The equipment enclosures consist of a concrete pad
surrounded by an 8-foot high chain -link fence with sight -obscuring slats. The air sparging
system consists of a series of 2-inch diameter vertical air sparging wells that are about 25 feet
deep. The well monuments are flush with the ground surface, or covered with sod.
No changes in the scope of the project have occurred since 1998 and no changes are proposed
under this permit application. The remediation equipment within the enclosures are no longer
operational. The primary system was shutdown based on research conclusions that the
operations have reached a practical end. The study indicated that these operations were not
decreasing the remaining TPH-D concentrations in the groundwater. There are no plans to
restart the system.
OPLC stated that this phase of operations is expected to take two years to complete, bringing the
site to closure also OPLC indicated that once the cleanup is complete and a closure agreement
has been accepted, the system will be decommissioned and the equipment enclosures and their
contents will be removed.
Conditions of Approval:
1. The site shall be left free of debris, litter, or other evidence of the temporary use upon
completion of removal of the use.

A fuel leak at the OPLC Maplewood block valve site circa 1985 resulted in a
series of environmental site assessments to assess the potential impacts of the incident.
The assessments indicated that weathered gasoline- and diesel -range petroleum
hydrocarbons from the leak were present in groundwater and in soil within the zone of
water table fluctuation beneath a portion of the surrounding neighborhood. OPLC
instituted a remediation program that, based on recent groundwater monitoring data, has
resulted in the reduction of the extent o f groundwater impacts at concentrations greater
than the Model Toxics Control Act [MTCA] Method A cleanup criteria.
Benzene concentrations in groundwater are provided in Figure 2. The plume of
gasoline -range petroleum hydrocarbons in excess of MTCA Method A criteria has
decreased to a small area around MVV-14 (Figure 3). No measurable free product has
been encountered during these monitoring events.
Diesel -range petroleum hydrocarbon impacts to groundwater above MTCA
Method A cleanup criteria is contained in three groundwater "hot spots". These pockets
are located near wells MW-25 to the west; wells MW-14, MW 16, and MW-34 in the
central portion of the site; and wells MW-1 S, MW-19, and MW-20 in the eastern portion
of the site (Figure 4). Monitoring wells around the periphery of these pockets have
demonstrated petroleum hydrocarbon values below Method A cleanup levels. This
observation is anticipated and consistent with continued degradation of the TPH-D
impact to the groundwater by chemical and biological.
The physical components of the remediation system include two equipment
enclosures constructed in November 1998. The enclosures are approximately 400 square
feet each and contain the aboveground treatment equipment. The northern equipment
enclosure is situated adjacent to the west side of the existing OPLC block valve
enclosure. The southern equipment enclosure is situated west of the existing playground
in Maplewood Park. The equipment enclosures consist of a concrete pad surrounded by
an 8-foot high chain -link fence with sight -obscuring slats.
The air sparging system consists of a series of 2-inch diameter vertical air
sparging wells located witbin the block valve property (at 3524 Southeast 5'h Street), the
north and west sides of Maplewood Park (3400 Southeast 61h Street) and next to the
eastern curb of Newport Avenue southeast (between 3330Newport Avenue Southeast and
3400 Southeast 6`h Street). The air sparging wells are about 25 feet deep. The well
monuments are flush with the ground surface, or covered with sod.
The vapor extraction wells are connected via shallow underground piping (2 to 4
feet below the ground surface) to air compressors situated in the above ground treatment
equipment enclosures. The underground piping is located beneath the following area: (1)
the southern margin of the block valve property, (2) Southeast 5t1' Street between the
block valve property and Maplewood Park, (3) the north and west sides of Maplewood
Park and (4) the grassed planter strip on the east side of Newport Avenue Southeast
(between 3330 Newport Avenue Southeast and 3400 Southeast 6`' Street) (Figure 5).
The remediation system described above began operating in December of 1998 and has
generally functioned continuously until March of 2006. Remediation equipment within
these enclosures is no longer operational. The primary system was shutdown based on
the conclusion that SVE operations at the site have reached a practical end point, and that

the remaining TPH-D concentrations in groundwater were not decreasing in response to
SVE operations but instead to natural attenuation. There is no plan to restart the system.
OPLC with Delta Consultants is currently working with the Department of
Ecology to develop a feasibility study to address the remaining "hot spots" of TPH-D
concentrations. This next phase of operations, estimated to require an additional 2 years
to complete, is expected to bring the site to closure. Once the cleanup is complete and a
closure argument has been accepted, the system will be decommissioned and the
equipment enclosures and their contents will be removed. Until that time, groundwater
monitoring will continue to be perforined throughout the neighborhood on a quarterly
basis.
A Temporary Use Permit is being requested to maintain the use of the existing
south and north equipment enclosures in the southeastern portion of the Maplewood Park,
and just north of Maplewood Park across SE 5`t' Street next to the Olympic Pipe Line
Company (OPLC) block valve enclosure (Figure 1). Both remediation equipment
enclosures are located in an R8 zoned area. The Temporary Use Permit LUA-03-084-TP
was issued on September 18, 2003. The pen -nit has expired; however, OPLC is
requesting continued operation of the remediation system at the site at least through 2010
to permit the development of a site closure argument.
