The Local Agency having complied,or hereby agreeing to comply,with the terms and conditions set forth in(1)Title 23, U.S.
Code Highways, (2) the regulations issued pursuant thereto, (3) Office of Management and Budget Circulars A-102 and
A-128,(4)the policies and procedures promulgated by the Washington State Department of Transportation and, (5)the Fed-
eral-aid Project Agreement entered into between the State and Federal Government, relative to the above project,the Wash-
ington State Department of Transportation will authorize the Local Agency to proceed on the project by a separate
notification. Federal funds which are to be obligated for the project may not exceed the amount shown herein on line s.,col-
umn(3)without written authority by the State,subject to the approval of the Federal Highway Administrator.All project costs
not reimbursed by the Federal Government shall be the responsibility of the Local Agency.

Name Monster Road Bridge Replacement and Roadway Approaches Length 600 feet approx.
Termini Monster Road Bridge
Description of Work
Replace the Monster Road Bridge over the Black River. This funding request is to prepare
a design report study, which includes surveying, mapping, preliminary geotechnical
exploration, bridge type, size and location, approach roadway alignments and construction
estimate. Will apply for PS&E funds after study is completed.

The Federal-aid participation rate in this project will be determined by the Federal Government.The parties expect that it will be 80 %,
however, it is understood that the rate may vary. The Local Agency agrees that this agreement is entered without relying upon any
representation by the State made outside of this contract,or contained herein,as to what the Federal participation rate will be.It further
agrees that it will not condition any future actions with respect to the project covered by this agreement upon past, current, or future
representations as to the Federal participation rate.The dollar amount of Federal participation cannot exceed the amount shown in line
s.,column(3).All costs not reimbursed by the Federal Government shall be the responsibility of the Local Agency.

Rcadway Width: 26 feet Number of Lanes: Two
The existing bridge consists of a 2-lane, 26 foot curb to curb roadway with no
: shoulders or sidewalk. Built in 1950, the bridge is comprised of a 3-span con;
crete mid-structure with timber approach spans on each end. In 1989 the north
: approach had two bent piles replaced and the entire deck overlaid with ACP. Ir
1991 a core sample from a north approach timber pile was taken and showed the
: center to be soft. The center concrete spans are designed to 1950 standards arid
:,-are vulnerable to earthquake forces The bridae currently has a 5-ton load limit

Dear Mr. Zimmerman:
We have received notification that this project has been completed. To expedite the
closure of our agreement with your agency,we have scheduled to close the project
accounts on June 8, 1999. If there are any expenditures incurred on this project that
your agency would like to claim,please process a final billing prior to this date.
Contact your Regional TransAid Engineer if you need an additional billing form.
If your agency is not able to prepare its final billing prior to the scheduled closure date,
please submit a written request for a time extension to the Regional TransAid Engineer.
No additional billings will be accepted after the scheduled closure date unless a time
extension has been approved.
