At the client's request, ESNW rep was onsite to observe or test cut slopes, test pH of stormwater, footing
excavations and earthwork activities.
Cut Slopes
While onsite, rep observed south slopes of the cut slopes to be near vertical and covered in plastic sheeting. Rep
observed that the east slopes of the excavation were near a 0.75:1 (H:V) slope. Although over -steepened, slopes
are stable in current configuration and stable to work within. The contractor is responsible for providing all safety
measures in accordance with state and federal regulation. Rep observed that contractor had sloped the southeast
corner of the footing excavation to a 1:1 (H:V).
Test pH of Stormwater
Rep took pH tests to determine the pH of stormwater. In the filled baker tanks rep observed 12.23 in a baker tank.
Rep observed that contractor added dry ice to lower the pH level. Rep observed that pH level was contained
onsite at a 6.5 pH.
Footings Excavations
Rep observed contractor use a medium sized excavator to excavate and the building footings. Rep recommended
and observed that contractor was over -excavating footings —4" from designed footing grade. Rep observed
contractor backfill the over -excavation with a Y-4" lift of 1 '/4' minus crushed rock for the footing subgrade to
increase stability during wall construction. Along the base of the footings, rep observed contractor install miradrain,
6" perforated pipe along the footings, and embed the pipe in a 12" lift of 2" wash rock.
ESNW rep will return to observe earthwork activities when requested by client

At the client's request, ESNW rep. was onsite to observe condition of cut slopes and test pH of stormwater and
footing excavation backfill. j
Cut Slope Observations:
While onsite, ESNW rep. observed the south slopes of the cut
slopes to be near vertical at a ratio of H0.75:V1. Although over -
steepened, slopes are stable in current configuration and stable
to work within. At the time of visit, no changes were observed in
the conditions of the cut slopes. The contractor is responsible for
providing all safety measures in accordance with state and
federal regulation.
Test pH of Stormwater:
While onsite, ESNW rep took pH tests to determine the pH of
stormwater. In the filled baker tanks, ESNW rep. observed <25
NTU in a baker tank. ESNW rep. observed the contractor added
dry ice to the baker tank to lower the pH level. ESNW rep.
observed that pH level was contained onsite.
Footing Excavation Backfill:
While onsite, ESNW rep. observed contractor bucket mixing
cement mix with saturated native fill material. Cement treated
footing excavation backfill was placed in approx. 12-inch loose
lifts and thoroughly compacted with a medium sized hoepack.
ESNW rep. will return as requested by client.

Supplementary Report:
At the client's request, ESNW rep was onsite to observe or test wall backfill with cement, cut slopes,
footing
excavations and earthwork activities.
Wall Backfill with Cement
While onsite, rep observed contractor use a medium sized excavator to mix and cement treat native brown silty
sand with gravel material. Rep observed contractor treat native material with a 3% cement mix. Rep observed
contractor place 12"-18" lifts for a hoe pack and 6"-8" lifts for a jumping jack during compaction. Rep took
compaction tests on backfill and observed material to be adequate of at least 95% of its maximum dry density that
was adjusted based on calculations with the cement treatment (130.7 lbs./cu. ft.).
Cut Slopes
Rep observed south slopes of the cut slopes to be near vertical and covered in plastic sheeting. Rep observed
that the east slopes of the excavation were near a 0.75A (H:V) slope. Although over -steepened, slopes are stable
in current configuration and stable to work within. The contractor is responsible for providing all safety measures
in accordance with state and federal regulation. Rep observed that contractor had sloped the southeast corner of
the footing excavation to a 1:1 (H:V).
Footings Excavations
Rep observed contractor use a medium sized excavator to excavate and the building footings. Rep
recommended and observed that contractor was over -excavating footings - 4" from designed footing grade. Rep
recommended that contractor backfill and place a Y-4" lift of clean crushed rock for the footing subgrade to
increase stability during wall construction.
ESNW rep will return to observe earthwork activities when requested by client
