Patrick,
Thank you for reaching out to me and I am in the process of working with staff to schedule the
preconstruction meeting for either next Monday 6/22 or Tuesday 6/23 based on availability. We are
currently furloughed on Friday's until at least the end of July which impacts our immediate
availability. Provided below is a list of items that are required for construction. Please note, many of the
items are already complete and I have noted in green that no further action is required. Items noted in
red still need completion or clarification. Items in purple can be finalized after construction begins but
are provided for continuity. If you could get me your contractor information today (at least the name
and contact) it would be greatly appreciated. Please let me know if you have any questions.
· Civil construction engineering and planning review mark-ups and final approval (plans have been
approved)
· Pre-construction meeting (looking at scheduling for 6/22 or 6/23 based on staff availability.)
· DOE NPDES Construction Stormwater General Permit (received and noted that the permit
coverage lasts until the end of the year)
· Authorization of special billing (provide an email with the name, address and phone number of
who will be the special billing contact/who pays the bills. This includes any overtime inspections,
if we need 3rd party consultants, use of City vehicles to sweep roads near the site etc.)
· Deeds of Dedication and REETA forms (not applicable)
· Business licenses for the contractor and subs (provide contractor name and contact information
and proof of licenses, including City business license)
· Insurance certificates (certificate shall name the City as additionally insured. Insurance
minimums and instructions can be found

· Performance bonds (provided by applicant and accepted. Please remember to mail the hard
copy.)
· Access, utility, and native growth easements as applicable (in process, will need to be finalized
prior to building occupancy)
· Stormwater covenants (in process, will need to be finalized prior to building occupancy)
· Provide a reference pdf of the Construction Stormwater Pollution Prevention Plan (CSWPPP) and
make sure one is onsite at all times. (Provide copy of CSWPPP to be onsite meeting DOE
requirements as reference)
· Please mail 2 full size hard copies of the approved plans attention Michael Sippo to City Hall
(provide)

Good morning Pam,
I have reviewed and approved the plans and am routing to the other departments for their signatures. I
will send the permit for signature once I get all the signatures. I have also routed the native growth
easement to our technical reviewer for any comments. Once I get those back will return to you along
with any comments/fill-in items.
Regarding the permit bond, below is a link to the permit bond form. The amount based off the bond
quantity worksheet of $193,471.85 is approved. You can wait to get the bond until after contractor
selection and before the preconstruction meeting. Please mail a hard copy of the bond to City Hall but
please also provide me a pdf copy. Here is a link to the City's permit bond

Mike,
I hope that you had a good long weekend.
I am attaching a draft of the easement. The surveyor has changed the easement exhibit to
match the required format. I would like to check on what project numbers should be referred to
and whether the language referring to an LUA should be changed, since this easement is due to
the method of stormwater dispersion instead of a land use requirement.

Good afternoon Pam,
Thank you for clarifying that the contractor will be selected following issuance of the Civil Construction
Permit. I will work early next week to get the permit issued so that you can move forward with
contractor selection. Please note that permit issuance does not allow construction to begin as there will
be requirements once your contractor is selected to hold a pre-construction meeting, obtain permit
bonds, insurance, business licenses etc.
I will review the covenant with this final submittal but based on my initial review I'm not anticipating any
changes. The covenant will be recorded prior to building occupancy. I will let you know who does the
recording whether it's the City or your team.
For the native growth retention easement I've copied my response from my May 20th email and attached
the easement document again. Please have your surveyor prepare the easement at your earliest
convenience as this will also need to be recorded prior to occupancy.
"Regarding the Native Growth Retention Easement, attached is our standard NGRE template. Please note
that the map exhibit must be in a recordable format as shown in the State instructions you sent over
which requires that it be on 8-1/2" x 14" or smaller and have a legible font size. This will likely result in
the map being on multiple pages or at least the map on one page and the line-work metes and bounds
descriptions on another. The legal description needed is for the parent parcel only and doesn't need to
be for the NGRE."

Mike,
Thanks for your detailed comments and help with getting this to permit. I appreciate the difficulty
of your reduced hours and plenty of work to do.
Habitat doesn’t have a contractor yet. They need the approved permit set, before they can sign
a contract with the contractor. They are very eager to get the permit, so they can proceed with
hiring the contractor.
The loose ends that I see are:
1. In coordinating our response to the retaining wall permit, we have a few new changes to
Sheet 9 (C3.0). Max is making those changes right now. It would be great if we could get that
sheet into the permit set and not have a later revision, although I don’t want to cause a delay in
getting the permit.
2. I am attaching what we have for the easement and covenant. Please let us know what we
need to do to complete it.

Good morning Pam,
Thank you for sending this over and all the great work that your team has done to get all of the
comments addressed. It's been very helpful throughout the entire process and I appreciate that you and
the team have been communicative and responsive.
I'll do my best to assemble the plans and approvals this week but did want to let you know that the CED
Department is furloughing Friday's until the end of July so I will have a reduction in bandwidth for the
foreseeable future.
Do you have a contractor selected at this time? If so, I would like to schedule a pre-construction meeting
for sometime next week. If not, then we can at least assemble all the approvals and pre-construction
documentation.
