Before we can accept the application as complete, the following information needs to be submitted: • A drawing depicting the depth of the water at 80' from the shoreline and the depth of the water at 126' from the shoreline. Winter and summer fluctuation should be shown on the drawing. • A map demonstrating the contours of the bottom and a written description of why the"L"dock will not be acceptable. • A plan demonstrating the length, shape, and width of the dock that was removed. • A plan demonstrating those docks in the general area that are the same length or longer than the dock proposed for the Baker's. • How many and by what method will the new pilings be installed? How will this equipment be brought to the site? • How will old pilings (if any) be removed? How will they be disposed of? • How will the disturbance to the lake bottom be addressed during the construction period? • From the description it appears that the dock will be used by at least three vessels. Is this correct? • It should be noted that the entire area is part of the Pacific flyway that is used by migratory birds. City staff will not this on the application. An Environmental Impact Statement (EIS) must be prepared for all proposals with probable significant adverse impacts on the quality of the environment. The purpose of this checklist is to provide information to help you and the agency identify impacts from your proposal (and to reduce or avoid impacts from the proposal, if it can be done) and to help the agency decide whether an EIS is required.
