Permit Issued? Sepa Review? Job Start Notice. 2. Traffic Control: In accordance with the Manual on Uniform Traffic Control Devices. Responsibility of permittee and contractor. Minimum signing – “Construction Ahead” each way. Flaggers: hard hats, vests, certified and signs. Must have current certification present and available for verification. Detours – approval by permittee; then submitted to inspector 5 working days in advance of Job Start Notification. Closures – informational signs (once approved by inspector) shall be placed 10 days in advance of closure. Notification of police, fire, school, METRO, etc. is the responsibility of the Permittee. Maintain one lane of open traffic at all times. 3. Backfill 36 inch minimum cover. Transverse cuts in Roadway - the entire trench shall be backfilled with 1 ¼ inch minus crushed surfacing base course meeting requirements of Section 9-03.9(3) of WSDOT Standard Specifications, and permanently patched within 30 days of initial excavation. All other cuts or trenching in roadway - Regardless of trench depth, native soil or import shall be utilized provided the material meets the requirements of 9-03.14(3) of the WSDOT Standard Specifications for Common Borrow. Backfill used for trenches exceeding 15 feet in depth will require a soil analysis prior to construction approval. Compaction: 95% compaction required entire depth 1 foot loose lifts with whacker 2 foot loose lifts with hoe pac Provide room for compaction around manholes and vaults. 4. No trenches open overnight: backfill, plate or fence. When using plates, the excavation must be shored; the plates pinned, shimmed and ramped with asphalt. Plates shall not be allowed to remain in place more than 72 hours without prior inspector approval. Steel Plate Ahead and Motorcycles Use Extreme Caution warning signs are required in accordance with the MUTCD. 5. Cold mix driving surface every night. 6. Cold mix sidewalks and driveways if directed. 7. Vacuum all saw cut slurry immediately. Centerline and fog line must be visible nightly. Maintain as necessary. Provide whatever measures necessary (compiling with all applicable local, state and federal regulations) to ensure adequate dust control. 8. Provide erosion control per plans or as directed by inspector. Maintain as necessary. Dewatering plan must be submitted and approved prior to Job Start Notification. 9. Keep storm drainage systems clean and functioning. Maintain as necessary. Permittee/Contractor is required to immediately notify KC Utility Inspection Unit, or assigned inspector of any damage to King County drainage systems. 10. Haul off spoils. No spoils shall be wasted on right of way. Haul to permitted site. This is the responsibility of utility and contractor to ensure strict compliance. 11. Reflective barricades are required at night on all temporary stock piles and parked equipment. Lighted barricades required on shoulder hazards. 12. Gravel shoulder restoration shall be a minimum 1 inch State Spec crushed surfacing top course, graded away from the edge of asphalt. DO NOT leave shoulders high. 13. Asphalt shoulders shall be cut at fog line and replaced full width or as directed by inspector. 14. Restoration of a trench within asphalt shall include a minimum of 6.5 inches of crushed surfacing material and HMA the same thickness as the existing asphalt pavement or a minimum of 2 inches, whichever is the greater. One foot trench cut (T-Cut) is required for all patching. Asphalt edge damage shall be trimmed and a minimum 18 inches wide patch. An overlay may be required. All patches SHALL be neatly trimmed, tacked and sealed. Without exception, open cut crossings shall be permanently patched within 30 days. 15. Overlays shall be a minimum of 2.0 inches of compacted HMA. Prior to the overlay, transverse joints and vertical curb lines shall be planed a minimum of 20 feet transverse and 6 feet from edge of curb. 16. Tie out all monuments prior to construction and replace them following overlay. All survey monuments along the proposed alignment shall be accompanied by a WA-DNR Permit. It is the responsibility of the permit holder to coordinate the adjustment of all valves, manholes, etc. during or following the overlay. 18. All new water meters or meter replacements shall be set at the property line unless a variance has been granted. Any abandonment of asbestos pipe shall require formal waiver from King County Road Engineer. 19. Replace all traffic stripes, buttons, crosswalks and traffic loops. All above ground facilities must comply with roadside obstacle standards. 21. If work is to occur on weekends, inspector must be notified by previous Thursday. 22.
