Club Prophet Systems Monthly User Fee Agreement

I acting on behalf of Maplewood Golf Course herein referred to as
Customer, hereby accept this proposal provided to me by Pro-Shop.Keeper Inc. herein referred to as Company.
Terms of Sale: In order for Pro-Sh opKeeper to reserve installation and training dates the widersigned agrees

Monthly Software Payments of $185 will then be due starting the 2nd month of your install and will
continue to be billed on a monthly basis.
Note: Either party may terminare this agreement with 30 days written notict;,.
Note: The $185 rental will remain constant as long as customer continues to use the CPS software in the .net
version. Any additional software modules requested will have an additional monthly rental fee.
�ustomer ResP!!Dgbllities:
Customer will be responsible for hiring a qualified networking company to install, setup, and cable the
necessary networking components of computers. This work is to include cabling, network configuration,
hubs/switches, computer installation, monitor and report printer setup, and all other standard "off the shelf"
peripherals. We will not support Pro-ShopKeeper on generic, home built PC's or on networks that do not meet
the specifications listed in 'Exhibit B'. tfyou are not purchasing computer from Pro-ShopKeeper, purchase
only industry standard, brand name PC's that meet our required specifications. Further, under no
circumstances will Pro-ShopKeeper neither be responsible for any network problems nor will they reimburse
Customer for any fees incurred by any O\ltside softwar,;/hardware vendors or Internet Service Providers. All
hardware warranty issues that arise from the purchase any hardware from PSK will fall under the
manufacturer's original warranty.
On-Site Training:
2 Days of omlte training are loduded with the monthly fee. Additional training hours may be required
based on the comple)(ity of the operation, software modules, the number of staff to be trained, and other
circumstances beyond our control. Additional on-site training can be scheduled thru Pro-Shopkeeper's sales
office and are available at a daily rate. (currently $800 per day)
All pass through upenses for travel, shipping, etc, See 'Exhibit A' that are usoeiated with the cost of
training •nd installation will be billed opon completion of lnltlal Installation and will be due for
payment within l O clays of the Invoice date. Travel Expenses are billed at cost and will not exceed
$1100,
