Customer is responsible to mark and/or cover all sprinkler heads. Notify crew

2Removal- Large
2 Hazard Permit $400.00
MTS to act in its capacity as your contractor/authorized agent to obtain necessary
permits as required by your local city or other governing body. MTS to submit
required documentation for removal application which may include removal
application form , site maps, replanting plans , traffic control requirements, and other
required documents . In order to remove potentially hazardous trees, additional site
visits and Tree Risk Assessments performed by a Qualified Tree Risk Assessor are
included if required by local ordinance .
Specific documents and processes for obtaining permits differ depending on the
city or governing body and it is important for property owners to be aware of what is
required within their area of jurisdiction. Specific replanting plans may be
necessary to include in the permitting process and should be discussed with MTS
prior to permit application .
Price listed is for processing of permit only, any fees or costs required by the city or
municipality will be paid by customer directly or included in final billing. MTS cannot
guarantee permits will be approved . Our fee will be charged to customer if permits
are filed by us , regardless of permit approval or related tree work.

*Job proposals or estimates are good for 45 days from date of bid.
*Property owner is responsible to obtain all permits and permissions on their property and/or neighbor access.

Customer responsible for all permits unless stated other wise on estimate.
**Customer must be present for any hedge trimming when the first edge cut is set. If customer chooses to not be present MTS
will not return to make adjustments to the hedge.
**Any dog or animal waste is to be cleaned up by customer before services are rendered. If this is not done MTS has a $100.00
charge to move animal waste and will not clean up fine debris from the project.
*Customer has 30 days to report any job discrepancies or damages. Matt's Tree Service will do all possible to preserve plants,
shrubbery, lawn (dents), driveways (concrete or blacktop) or underground utility systems (water, septic, gas, irrigation or invisible
fences) and will not be held liable. Matt's Tree Service has the first right/option for any repair in case of damage.
*All trees to be cut are designated and authorized by signer of contract. Signer assumes full responsibility and holds Matt's Tree
Service harmless from prosecution. Customer is responsible to mark trees if not available day of service.
*It is customer’s responsibility to inform the Matt's Tree Service of greenbelts, underground lines and utility systems, protected tree
status, property lines, irrigation systems, electric fences, etc.
Customer is responsible to mark and/or cover all sprinkler heads and notify crew lead/crew where irrigation lines lay.
*The best judgement by Matt's Tree Service is used for pruning, trimming, shaping, and thinning unless otherwise instructed the
customer. Crew will stop cutting if objects like nails or spikes are discovered in any trees. Cutting continues after reassessment
and/or cost adjustment.
Liability for Matt's Tree Service is limited to job total. Customer agrees to pay attorney fees for both parties for any dispute or
disagreement of this contract.
