ON-CALL WORK ORDER ADDENDUM NO. 1

ON-CALL TREE MAINT ENANCE, PLANTING AND REMOVAL SERVICES

This ON-CALL WORK ORDER ADDENDUM #1 is made and entered in to on May 27th
2022, betw een the City of Renton, (City) and Root Cause LLC
(Contractor) collectively (Parties) pursuant to and in compliance with the MASTER AGREEMENT
FOR ON-CALL TREE MAINTENANCE AND REMOVAL SERVICES CAG-22-188 dated 25th May, 2022
(Agreement) following bid proposal process.
The Parties agree as follows:
Each and every provision of the MASTER AGREEMENT FOR ON-CALL TREE MAINTENANCE AND
REMOVAL SERVICES CAG-22-2022 dated May 25th, 2022 (Agreement) shall remain in full force
and effect and the CAG-22-188 ON-CALL WORK ORDER ADDENDUM NO. 1 is as follows:
1.Project Name: Downtown Renton Spring tree replacement 2022
2.City Division: Urban Forestry & Natural Resources

3.Scope of Services:
Planting of trees in various locations
(And as set forth in CAG-22-188 ON-CALL AGREEMENT Exhibit A)
4.Technical Approach: As per scope of work in contract
(Additional specifications and requirements may be attached to this form or as set
forth in CAG-22-188 ON-CALL AGREEMENT)
5.Deliverables: Tree planting work as invoiced
(As set forth in CAG-22-188 ON-CALL AGREEMENT Exhibit B)

6.Schedule:
(As set forth in CAG-22-188 ON-CALL WORK ORDER ADDENDUM #1)
7.Cost/Hour Estimates:
Contractor agrees to perform the services as described above for the amount not to exceed
$7,244.58 unless modified by the City in a signed written subsequent addendum
8.Due Date or Completion Date: 05/27/2022
9.Total Amount Payable to the Contractor under the Maser On-Call Agreement is
summarized as follows:
Original Non-Exclusive MASTER AGREEMENT FOR ON-CALL TREE MAINTENANCE AND
REMOVAL SERVICES CAG-22-188 authorized amount not to exceed per the duration of the
Agreement:
$35,000.00
