Collection/Liens/Bond Claims This proposal shall serve notice of our intent to place liens on any private property improved upon should payment not be received in full upon completion of contracted work • Bonded-project owners agree to furnish any bonding information upon request, and to cooperate fully with Puget Paving’s efforts to collect payment via liens or bond claims if necessary • This proposal may be withdrawn by Puget Paving if not accepted within 30 days of submittal. EQUIPMENT/TRUCK RENTAL Puget Paving has a minimum truck, labor, or equipment rental period of 4 hours. Rental time in excess of 4 hours is charged in 15 minute segments and rental time will be rounded to the nearest 15-minute increment. Rental rates are available upon request. Rental rates update yearly and are not required to be furnished to customer prior to rental time being charged INSURANCE Puget Paving carries the following liability insurance coverage: General Liability: $1,000,000 Occurrence / $2,000,000 Aggregate / Deductible: $250 • Blanket Additional Insured Products/Completed Operations: CG8583 04/13 • Primary and Non-Contributory and Waiver of Subrogation / General Liability Extension Endorsement: CG8810 04/13 • Per Projects General Aggregate: CG8870 12/08 Auto Liability: $1,000,000 Combined Single Limit • Blanket Additional Insured, Waiver of Subrogation Auto Coverage Enhancement: CA8810 01/10 Umbrella: $5,000,000 Limit / $10,000 Self Insurance Retention / Occurrence Form Pollution Liability: $2,000,000 Occurrence / $2,000,000 Aggregate / Deductible: $10,000 Any contractual requirement to carry more insurance than that listed above may require an additional charge over and above this proposal Asphalt grinders have a minimum rental period of 4 hours • It is the responsibility of the project owner/prime contractor to notify Puget Paving a minimum of 72 hours in advance of any asphalt milling work to occur greater than 6” depth; Washington State law requires that Puget Paving call in its own locating ticket when grinding in excess of 6” depth and will not grind in excess of that depth without valid one-call locates • Unit- or lump-sum pricing for grinding may change as the result of a field request to grind deeper than quoted pricing • Unit- or lump-sum pricing may change as the result of under-trucked grinding operations when Puget Paving is not responsible for truck scheduling CIVIL/UNDERGROUND UTILITIES Unless otherwise specified, pricing does not include dewatering, export/disposal of unsuitable soils, import of structural fill, survey staking or engineering, resolution of unknown underground utility conflicts, bonding, permitting, or testing I have read and understand the above described clarifications and exclusions and agree to include them in any contract I enter into with Puget Paving and Construction, Inc. Subgrades, if not prepared by Puget Paving, shall be firm and unyielding and graded to drain at 1% slope minimum to qualify for any warranty • Design grades at less than 1% slope do not qualify for drainage warranty • Grades prepared by others are subject to inspection by Puget Paving staff and, if found defective, an additional release and waiver may be required prior to paving • Puget Paving cannot be responsible for subgrade failures that result from any reason other than negligence on the part of Puget Paving • If Puget Paving paves or imports crushed rock on subgrade prepared by others and exceeds the asphalt or crushed rock tonnage quantity specified by more than 105% of the planned quantity, and if that overage was the result of poorly prepared or soft subgrade it may result in an additional charge • Puget Paving reserves the right to not proceed with paving on a scheduled paving day if weather on that day and location (precipitation/temperature) violates current WSDOT specifications--specifically, WSDOT specs disallow any asphalt paving at less than 42 deg F for 2” lifts and 35 deg F for 3” lifts and through any amount of standing water • Precipitation limitations apply depending on severity • If a customer/prime contractor demands that paving proceed in violation of weather specifications, an additional release and waiver of warranty may be required prior to commencement of any paving FINE GRADING AND ASPHALT PAVING
