HomeMy WebLinkAboutAddendum - 1AMENDMENT NO. _1_ TO 2021-2022 HUMAN SERVICES AGREEMENT CAG# 21-046 THIS AMENDMENT, dated for reference purposes only as September 6, 2022, is by and between the City of Renton (the “City”), a Washington municipal corporation, and Renton Ecumenical Association of Churches (REACH) (“Agency”), a Washington nonprofit corporation. The City and the Agency are referred to collectively in this Amendment as the “Parties.” Once fully executed by the Parties, this Amendment is effective as of the last date signed by both parties. Whereas, the City engaged the services of the Agency under Agreement CAG-21-046, dated March 16, 2021 to provide necessary services for the Center of Hope shelter (referred to herein as the “Agreement”); Whereas, the Parties wish to amend the Agreement to increase the compensation in order to provide additional services to Renton residents. NOW THEREFORE, It is mutually agreed upon that CAG-21-046 is amended as follows: 1. Scope of Work: Section 1, Scope of Work, in Exhibit A is amended to increase the 2022 scope of work for case management sessions as described below. 2022 Amended Service Units 1st Qtr 2022 2nd Qtr 2022 3rd Qtr 2022 4th Qtr 2022 2022 Totals Unduplicated City of Renton clients served 7 6 6 6 25 Number of bednights provided to Renton residents. 287 286 286 286 1,145 Number of case management sessions provided to Renton residents (15 min. increments) 100 100 100 100 400 In 2021, the location of the Center of Hope Shelter changed to Lakeridge Lutheran Church at 7465 S. 112th St., Seattle, WA 98178. The number of shelter beds available per night there is currently limited to 35. 2. Compensation: Section 4, Compensation, is amended so that the maximum amount of compensation payable to Consultant is increased by $60,000 from $7,500 to $67,500 for 2022. Compensation shall be for Agency staff personnel costs from January 1, 2022 to        CAG-21-046, Adden #1-22 PAGE 2 OF 3 December 31, 2022 for Center of Hope shelter program services. Such staff includes, but is not limited to shelter support staff, Program Coordinator, Program Manager, and Executive Director.Copies of payroll backupdocumentation shall be submitted to support invoices. Funding Amount Funding Source 2022 Original Amount $ 7,500 General Fund Amend. No 1 increase $60,000 General Fund – Rev. Replace. Revised Total for 2022 $67,500 3. Notices: Section 16, is amended to change the City of Renton contact to: Lori Fleming Human Services Coordinator 1055 South Grady Way, 1st floor Renton, WA 98057 425-430-6655 Lfleming@rentonwa.gov 4.All terms of the Agreement not explicitly modified herein shall remain in full force and effect and such terms shall apply to Work performed according to this Amendment as if fully set forth herein.
