Description of Work
!t6th Avenue SE,classi6ed as a minor arterial,was annexed to the City of Rentoa in 200g.It generaily has two ianes for traffic
with le8-turn lanes ae intersections in the current configuration. Sidewalks are intermittent.
In order to enfianee vehicular,bicycle,and pedestrian safery along this eocridor,this praject will widen the roadway ta inciude bike
Ianes,a two-way-teft-lane,new pavement,curt�s,gutters,sidewatks,street lights,traffic signa3s,stonn water drainage,striping and
tandseaping,from Puget Drive SF,to the south City Limits.

and executed on 1112o1zoj 5 and identified as Agreement Mo. CAG-i 5-221 ,
Af1 provisions in the basic agreement remain in effect excep#as expressly madified by this s�pplement.
The changes to the agreement are described as fallows:

Section 1,SCOPE OF WORK, is hereby changed to read:
This scope of work is amended to add support serviees for the ongaing environmental compliance reviews of the project during
cons[ructian and provide weekly oversight to assure contractor comptiance with Washington State water quality standards and the
King County permit.
