Description of Work
Environmental Services for South 7th Street & Shattuck Intersection Improvements.
The Local Agency of City of Renton
desires to supplement the agreement entered into with Widener&Associates
and executed on 05/09/2015 and identified as Agreement No. CAG-14-075
All provisions in the basic agreement remain in effect except as expressly modified by this supplement.
The changes to the agreement are described as follows:
I
Section 1, SCOPE OF WORK, is hereby changed to read:
see attached scope of work for the construction support services.
II
Section IV, TIME FOR BEGINNING AND COMPLETION, is amended to change the number of calendar days for
completion of the work to read: New completion Date 06/30/2016

Section V, PAYMENT, shall be amended as follows:
Authorization maximum amount payable shall increase by$15,705.20 to a new maxium payable amount of$27,439.40,
as set forth in the attached Exhibit B-Supplement No.1.
as set forth in the attached Exhibit A, and by this reference made a part of this supplement.
If you concur with this supplement and agree to the changes as stated above, please sign in the appropriate spaces
below and return to this office for final action.

Environmental Scope of Work
South 7th & Shattuck Project
City of Renton
Construction Support
The Consultant will provide construction support by inspecting the project as
needed. Anticipated construction support includes:
• Assist in pre-award activities (bid result analysis)
• Answering technical questions as required
• Responding to RFI's during construction of this project
• Addressing Corp Permit questions/issues
• Coordination required with any permit changes
Deliverables:
• Monthly progress reports to accompany invoices.
• Inspection reports as needed.
• Attending coordination meetings
