Brief description of the proposed contract change(s) and locations(s). This change order consists of the following: 1. Delete Bid Item 1 -22 "Precast Junction Box" at original, bid price of $6,400.00 per each. 2. Delete Bid Item 2 -33 "Precast Junction Box" at original bid price of $6,500.00 per each. 3. Add Bid Item CO -6 -1 "Precast Junction Box" replacing: Bid Item 1 -22 at revised unit price of $4,900.00 per each. 4. Add Bid Item CO -6 -2 "Precast Junction Box" replacing Bid Item 2 -33 at revised unit price of $5,000.00 per each. RFI #001 (attached) was issued on May 29, 2013 documenting these revisions. 2. Reason(s) for the change(s) (Continue on reverse if necessary). On 29- May -2013, ICON submitted RFI #1 regarding the need for aircraft rated strength for the precast communication!., junction boxes on the communications duct. All precast junction boxes were determined to be located outside of the Taxiway Safety Area (TSA), therefore boxes rated to meet WSDOT standard H -20 loading may be used. Because the H -20 loading boxes are to a reduced strength, a credit for these items was negotiated. 3. Justifications for unit prices or total cost: Justification was prepared in accordance with FAA: program guidance letter 12 -03. 4. The sponsor's share of this cost is available from: Port Funds 5.
