CONTRACT CHANGE ORDER AGREEMENT
CONTRACT: 2013 Street Patch and Overlay with Curb Ramps CAG-13-103
CONTRACTOR: Lakeside Industries
SUMMARY OF PROPOSED CHANGE:
Reason/Justification: The contract called for 3 loops to be installed on NE 10th St.. After the contract was awarded,it was noticed that the
home runs from the loops on Sunset Blvd NE were cut during the grinding of NE 10th St. Also after the contract was awarded,a thin King County
overlay on Renton Ave S has started to delaminate and is in need of a grind and overlay. Lakeside has agreed to use the contract bid items for
these extras but requested new bid items for additional Mobilization and Traffic Control.
Contract Change:Add 7 induction loops,and add grind and overlay to a portion of Renton Ave S,including additional Mobilization and Traffic
Control.
Add the followinp quantities to bid item plan quantities in the schedule of prices.
