SUMMARY OF PROPOSED CHANGE:
CO -2 Remove and Dispose of Buried Heating Oil Tank (on Schedule B) LS
On December 18, 2013 the contractor encountered a buried residential heating oil tank at 679 Camas Ave NE
while excavating to replace the sewer tee for Schedule B, Bid Item B -01. The contractor contacted a heating
oil tank removal company to obtain a Residential Underground Heating Oil Tank Removal permit from the City
and to dispose of the tank. The December 20th letter from R.L. Alia Company gives the costs for removal and
disposal as $1, 039.39.
Remove and Dispose of Oil Tank lump sum at $1,039.39 = $1,039.39
9.5 % sales Tax = $98.74
TOTAL CHANGE ORDER COST = $1,138.13

Alia encountered an unknown oil tank on 12/18/13. Alia, excavated, removed and loaded it out to a certified
tank removal contractor.
As you can see per the attached spreadsheet, the cost to perform this added work comes to $1,039.39.

The work was done in accordance with local and state regulations. Tanks By Dallas has
made all means to leave the site as close as possible to its original condition. Tanks By
Dallas is not liable for any underground conditions at this time or any in the future.
Any and atl know underground conditions and any potential unknown underground
conditions have been property conveyed to the person authorizing the tank work. This
certificate is the result of a binding agreement/ contract produced prior to
commencement of the tank decommissioning.
"The ICC certified UST decommissioner noted above is responsible for notifying the person authorizing
the work of a confirmed petroleum release, leak or spill. As the responsible party authorizing the work,
said party must have responsibilities under the law to report petroleum release, leaks or spills to the
appropriate state or federal agencies.
