RIVERVIEW PARK— BRIDGE REPLACEMENT

ADDENDUM TO THE DESIGN CONSULTANT AGREEMENT
BETWEEN PND ENGINEERS, INC.
AND THE CITY OF RENTON, WASHINGTON
This addendum is entered into between the City of Renton and PND Engineers, Inc.
amends and attaches to and is part of the existing agreement entered into previously
between the parties on December 27, 2013.
Amendment 2: Article 4, Compensation:
Pursuant to section 4.4 of this Agreement, entitled Change Order, is amended as
follows:
The total payment for Addendum No. 5 shall be on a Time and Materials basis, with a
not to exceed limit of Fifty-Five Thousand Six Hundred and Thirty-Five Dollars
($55,635.00).The total amended contract amount shall not exceed Three Hundred
Sixteen Thousand Eight Hundred Ninety-Six Dollars and Twenty Cents ($316,896.20),
including WSST.The total reflects the following additional work:
• Additional fabrication inspections by PND and their sub-consultant Otto Rosenau
and Associates
• Preparation of Kick Plate Modification Change Order
• Discussions and meeting regarding quality control issues by bridge fabricator
• Review of documentation from bridge fabricator
• Additional bridge inspections prior to and including bridge installation
• Preparation of As-Built Drawings
In all other respects,the Scope of Work/Basic Services; City Responsibilities; Added
Services; Reimbursable Expenses; Insurance Requirements;Termination of Agreement;
Successors and Assigns; Non-Discrimination and Compliance with Federal, State and
Local Laws;Arbitration; and Hold Harmless; and exhibits to the agreement shall remain
the same as agreed to in the original agreement.
This agreement is entered into as of the day and year written below.

Amendment No.5
Additional Construction Inspection Services
September 10,2015
(A) SCOPE OF SERVICES:
INTRODUCTION
This work order is issued pursuant to the Standard Agreement for Professional Services dated December 27,
2014 between the City of Renton and PND Engineers,Inc. Unless otherwise specified below,the performance
of services hereunder and the payment therefore shall be subject to the terms and conditions of said Agreement.
The fees shown are estimates and actual fees and expenses including mark-up will be submitted monthly for
invoicing.
SCOPE OF WORK
This amendment identifies additional costs incurred as a result of the continued duration of fabrication at NW
Docks as well as changes to the original design,unanticipated quality control discussions with NW Docks and
additional third party testing.Specific items included are as follows:
1. Additional fabrication inspections for PND subconsultant Otto Rosenau and Associates which have already
been performed. These additional inspections were recommended by PND and endorsed by the City of
Renton.
2. Otto Rosenau Inspection to complete the project
a. Fabrication Inspection—three inspection per week(Estimate 6 total)
b. Dye Penetrant Testing on 30°o of Bridge Welds(Estimate 2 inspection trips)
c. Two Additional Inspections are included as a contingency.
d. Final Concrete Placement Inspection
3. Preparation and discussion of the Kick Plate Modification Change Order.
4. Preparation of Documentation regarding Quality Control issues at NW Docks.
5. Discussions and Meeting regarding Quality Control issues at NW Docks.
6. Review of documentation from NW Docks (revised\PPS's and WPQ's).
7. Review of NDE testing performed by all parties at NW Docks.
8. Additional Fabrication Inspection(Summarized below)
a. Inspection to verify on-site QC list of welding issues.
b. Twice weekly inspection at NW Docks(Estimate 4 total)
c. Final Inspection prior to Leaving NW Docks
d. Two additional inspections are included as a contingency.
9. Bridge Lift Inspection(Three engineers from PND on-site)
10. Substantial Completion Inspection
11. Final Concrete Placement Inspection
12. Preparation of As-Built Drawings. Please note that these were in the original scope of work,however the
budget for this task was consumed by the increased number of fabrication inspections.
ASSUMPTIONS
• This estimate includes Engineer's time to supervise field staff and review daily field reports,mileage for
traveling to the site,staff support,and administration.
• The estimate should be considered to be approximate because our level of effort and actual time of
involvement will be influenced by factors that are not under our direct control. These include the
contractor's actual time of performance,efficiency of the crews,the weather,and the actual number site

visits required. If the level of effort for construction observation services described herein exceeds this
estimate we will notify the City's representative. We will also notify the City's representative if conditions
are encountered that require additional modification to the scope of services. We will not proceed with
any modification without the City's prior authorization. If additional site visits are required,they will be
charged on a time and materials basis at the requested level of effort.
• PND's construction services do not include supervision or direction of the actual work performed by
the contractor, the contractor's employees,or agents. Our professional opinions and conclusions will
be made in accordance with generally accepted engineering principles and practices. However,we will
not undertake the guarantee of the construction, nor will our testing and/or observations relieve the
contractor of their primary responsibility to produce a completed project conforming to the project
plans and specifications. In addition,PND will not be responsible for job or site safety on this project.
DELIVERABLES
• Daily Field Reports
• Special Inspection Submittal to the City of Renton Building Department
SCHEDULE
Following Notice to Proceed by the Client,PND agrees to perform the above-described services and to diligently
pursue the project and make every reasonable effort to finish all items in a timely manner.
FEES
PND will provide these services on a time and materials basis for a maximum budget of$55,635 as detailed in
the cost proposal.Work will be performed according to the attached rate schedule.
