SUMMARY OF PROPOSED CHANGE:
Provide all labor, equipment, subcontracting, bonding, insurance for the complete work required for the extra
work as outlined below:

ITEM A01: Electrical Equipment Shelter—Pre-Manufactured LS 1 $8,500.00 $8,500.00
As part of the original bid, the City solicited bids on two alternate items
(A01 &A02). These items were not included in the"Total Bid Price"for
the determination of the low bidder and the award of the contract.
While the project was under construction,the contractor was authorized
to proceed with the construction of"Alternate Bid Item A01 —Electrical
Equipment Shelter—Pre-Manufactured"per the specifications and at
the bid price provided.
Total ITEM A01 $8,500.00
Sales Tax(9.5%) $807.50
Total Additional Work $9,307.50
