SUMMARY OF PROPOSED CHANGE:
Reason /Justification: The original design included in the Contract Plans did not include provision for installation of the j -boxes on the
east side of Garden Avenue N. at the base of the slope, flush with the top of curb. This change order establishes payment under force
account for the Contractor to install a Type 1 junction box protection pad and curb wings at luminaires #7 through #13 on the east side
of Garden Ave. N. The pad and curbs shall be constructed as shown in the attached Type 1 Junction Box Protection Detail and Field
Directive #11.
Contract Change: This work is to be performed on a force account basis with complete force account sheets signed by the City's
representative on the day the work occurs. The Contractor shall submit forcre account sheets, corresponding invoices and required
supporting documentation.
