Storm&Water System Improvement Project C.O.# 4 (CAG-11-030) CONTRACTOR: KC Equipment,LLC SUMMARY OF PROPOSED CHANGES: Item #A55-CO #4: The contractor installed bio-degradable erosion mats and berms to the bioswale area and cut out plant wells in the mats to accommodate bio-swale planting and reduce erosion off the Swale slope during heavy rains. The contractor is requesting an additional $5,000 for materials, labor and equipment to install the erosion control structures. Item #625-CO#4: At station 20+57 a conflict with the existing fiber optic&installed storm pipe required the new 12-inch ductile iron pipe to need two vertical bends, shackling and concrete blocking at the bends. Approximately four hours of down time was required to determine the course of action and included one laborer, one operator, a flagger and an excavator on standby until the City was able to determine what was needed and to respond/solve the discrepancy in the design plans. The contractor is requesting an additional $2,500 for labor, equipment and material costs to install this vertical deflection to avoid the new storm pipe with adequate vertical separation between the utilities. Item #B26-CO#4: At station 11+10 there was a conflict between the fiber optic, gas and the proposed 6- inch water main to connect with the proposed fire hydrant. The contractor installed two additional bends in order to pass under the existing fiber optic and gas mains. The contractor is requesting an additional $1,500 for the additional shackling work, labor,concrete, and pipe bend materials needed to deflect this pipe beyond the original work for just pipe and trenching.
