This supplement adds to design support services during construction to prepare design of bridge modifications requested by the City after the project was bid. The Local Agency of City of Renton desires to supplement the agreement entered into with Otak,Inc. and executed on 3/23/2011 and identified as Agreement No. CAG-11-043 All provisions in the basic agreement remain in effect except as expressly modified by this supplement. The changes to the agreement are described as follows: 1 Section 1, SCOPE OF WORK, is hereby changed to read: Work as described in Exhibit A-1, dated 4/7/7014. I I Section IV, TIME FOR BEGINNING AND COMPLETION, is amended to change the number of calendar days for completion of the work to read: Decemher 31, 2016 III Section V, PAYMENT, shall be amended as follows: An increase in hudget of%]5,600 00 as described in Exhibit E,-1, dater) 4/7/2014_ as set forth in the attached Exhibit A, and by this reference made a part of this supplement. The following scope of work covers the design modification work and printing costs for construction documents,not included in previous supplements. 17.0 Design Services during Construction • Revise the design of the bridge modifications,including initial sketches and coordination with WSDOT to determine viability of proposed revision;revised plans, specifications, and quantities;and evaluation of costs for the revision. • Printing costs for the conformed set assuming 25 sets of half-size plans and specifications, and 5 sets of full-size plans.
