CONTRACT: CITYWIDE WALKWAY IMPROVEMENTS/SIDEWALK REHAB&REPLACEMENT 2011 CAG-11--149
CONTRACTOR: Tri-State Construction,Inc.
SUMMARY OF PROPOSED CHANGE:
Reason/Justification:Original design of the Maplewood(Schedule A)portion of the project resulted in numerous driveway approach
grades which exceeded allowable slopes for vehicle passage over the driveways.It was necessary to redesign the entire affected area
by raising the planned elevation of the new curb&gutter,thus reducing the slopes on the driveway approaches.Changing the curb&
gutter elevations changed the gutter low spots and drainage patterns,requiring redesign of the storm drain system which added over
100 LF of storm drain pipe and several catch basins and other features.Because four(4)driveway approaches and 720 LF of curb&
gutter had already been placed and were no longer at the correct elevations,they would have to be removed and replaced with new
ones at the revised elevations.
Contract Change:On Schedule A remove and replace 720 LF of already-placed curb&gutter and driveway approaches 3 on RD-03
and 1*,3 and 5 on RD-04 as shown on attached Removal sheets for RD-02,RD-03 and RD-04.Revise other(not already-placed)curb
&gutter,sidewalk and driveway approach elevations and other features,and make deletions and additions to storm drain lines,catch
basins(CBs)and drainage features as shown on attached Removal sheets for RD-02,RD-03 and RD-04,and revised finish plan
sheets RD-02,RD-03 and RD-04,DT-02 and cross-section sheets XS-01 and XS-02.
*Driveway approach 1 on RD-04 may be left as is-decision to be made in field.
Deduct the following quantities to bid item plan quantities in the schedule of prices.
