Page 1 of 3 CADocuments and Settings\kfaasse\Local Settings\Temporary Internet Files\Content.Outlook\JBH19U3H\Change Order No 14 Cover.docx Change Order #014 Contract Number: CAG-11-192 Federal Aid#:STPUL-CM0167(048) Date: December 5, 2012 Page 2 of 3 THE CONTRACT IS MODIFIED AS FOLLOWS: Background Per the original plan design,the Joint Utility Trench (JUT)was to be installed under the BNSF Railroad trestle at depth of approximately 4'to 6'.This section of JUT would also go through possible contaminated soils and beat a depth where dewatering would be highly likely. To avoid these contaminated soils and other potential utility conflicts, it was decided by the Utilities, City, and the Contractor,that the best alternative would be to shallow the JUT to a minimum depth 16". So as to avoid delay through the design process and to be able to better handle potential conflicts in the field,it was decided by all parties,that a design build approach tracked via Force Account was preferred. Description of Change Per Field Work Directive 006,the joint utility trench will be field fit from approximately 31+00 to 33+50.The installation of the trench was per the requirements of Puget Sound Energy's"Trench Dimensions 6790.0130",page 3 and 4,dated 7/1/2011. The installation of conduit and backfill for this section of trench was tracked via the applicable bid items and has been paid to date.All extra work associated with this design built change was approved and tracked by the Construction Management field staff according to WSDOT Standard Specification 1-09.6.All work, materials,and measurements were in accordance with the provisions of the Contract for the type of construction involved, unless stated otherwise in this Change Order. Description of Work Field fit the installation of the joint utility trench to the west side of the existing sidewalk from approximately STA.31+00 to approximately STA 33+50, as discussed during the 7/9/2012 weekly progress meeting.In areas where a minimum cover of 12-inches is installed, provide 3000 psi full strength concrete encasement. The installation of the trench was per the requirements of Puget Sound Energy's"Trench Dimensions 6790.0130", page 3 and 4, dated 7/1/2011. Minimum separation in this area shall be 6-inches. Per the direction of the Resident Engineer,the Contractor located and excavated an existing power vault before installation of this section of trench began. In addition,a new 12"water main and storm drain had to be installed under the now shallow JUT,whereas the original design was called for the water main and storm drain above this section of JUT.The additional time to go make this adjustment was tracked via Force Account. Material 32 cubic yards of 3000 psi Full Strength Concrete for encasement. Plans/Reference Puget Sound Energy's"Trench Dimensions 6790.0130",page 3 and 4,dated 7/1/2011. Timeline The original design per the Contract plan set was to install the joint utility trench at a depth of approximately 4'to 6'from STA 31+00 to 33+50.The time to construct this portion of the Joint Utility Trench was offset by eliminating the proposed deeper trench section,eliminating off-haul of contaminated soils,and the elimination of dewatering activities. No time impacts were realized for this scope of work. A total of—0—additional working days will be included with this change order.
