COST MODIFICATION:Fifteen Thousand One Hundred Thirty-nine dol(ars and Forty-seven cents$15,139.47(which includes
State of Washington Sales Tax)added to the original construction contract amount: the sum of Forty-eight Thousand Eight
Hundred Eighty-six Dollars and Fifly Cents ($48,886.50)(which incWdes State of Washington Sales Tax). All other conditions of
the original contract are a part of this change order. The payments and/or additional time specified and agreed to in this change
order include every claim by the contractor for any extra payment or extension of time with respect to the work described herein,
including delays to the overall project. The work covered by this change order must be performed under the same terms and
conditions as that included in the original contract.

Reason for Change: Owner Request(X) Unforeseen Q Other O Specify:Remove additional sections of damaged
concrete adjacent to walks originally designated in the Scope of Work;to incorporate drinking fountain and telephone pads for a
uniform appearance and;to install utility sleeves beneath the new sidewalks for future use. Amount includes thickened concrete to
match existing where it occurs,re-bar for reinforcement of all sections,demolition,4-inch PVC conduit,concrete and other
materials to match the original Scope of Work.
