and executed on 10/5/2009 and identified as Agreement No, CAG 09-170
All provisions in the basic agreement remain in effect except as expressly modified by this supplement.
The changes to the agreement are described as follows:

Construction Services—Project Oversight and Close-Out
KPG Inc
Scope of Work
A. Project Description
The purpose of this supplement is to perform project close-out activities for the Rainier Avenue S
Project. These activities include the following:
• Continued project oversight and construction documentation support for the remainder of the
construction contract.
• Preparation, filing, and archiving of the required project paperwork generated during the
project oversight effort.
• Assistance in project filing and archiving of previously generated construction documents.
Change Order support.
This scope and fee will serve as Supplement No. 6 of CAG-09-170. The Consultant Fee Determination is
attached to this scope of work and is labeled Exhibit E-1.
B. Deliverables
Consultant deliverables are described in Task 14 of the Scope of Work.
C. Proposed Schedule
This supplement will extend the current contract completion date of December 3111,2014 to March 311C,
2015.
D. Scope of Work
Task 14—Construction Services
This task covers the effort required to manage the contract and assure that the project meets the City's
expectations for schedule,budget and quality of product. Effort included under this task is as follows:

14.7 Project Oversight—The consultant shall continue project oversight duties that include, but are
not limited to: the preparation of DBE Paperwork, Contractor Payrolls, Weekly Statement of
Working Days, Pay Estimates, Environmental Permitting Closeout and other Project Closeout
Paperwork. This task also covers any field walkthroughs required during the remainder of Plant
Establishment Phase of the contract.
14.8 Project Filing and Archiving—As directed by the City,the consultant shall provide support to the
City in organizing project files for archiving. This includes the remainder of the ROW flies and
the paperwork identified in Tasks 14.7 and 14.9.
14.9 Change Order Support—As directed by the City,the consultant shall provide support during the
discussions, negotiations, and preparation of a Change Order currently under review with the
City and JEL
Deliverables:
• Construction Documents generated through the project close-out effort and Filed.
