NOTICE FOR ALL NEW PLATS AND SHORT PLATS
The Post Office wants to be involved in helping you locate your mailboxes before construction begins. Please call 1 -800-
275-8777 for the location of the Post Office that will service your plat. Then, take a copy of your plat map along with
this form to the specified Post Office for their sign-off.
Please submit the signed form along with your application.
