ADDENQUM NQ. 2 T4 CONSULTANT AGREEMENT

ORIGINAL CONTRACT DATE: Septembe�15, 2Q14
This Addendum amends the above-refe�enced agreement {the "Agreement"). All
provisions of the Agreement that are not inconsistent with this Addendum shall remain in effect.
For valuable consideration and by mutual consent af the parties, ti�e Agreement is
modified as follows:
1. The "Completifln Date" in the Agreement's page one head'rng is hereby modified
to read "12/31/2018" (December 3Z, 2018).
2. Consultant agrees to waive any protest it may have regarding this Addendum and
acknowledges and accepts that this Addendum constitutes final settlement of all claims of any
kind or nature arising from or connected with any work either covered or affected by this
Addendum. This Addendum, unless otherwise pravided, daes nat relieve the Cansultant from
strict compliance with any and all guarantee and warranty p�ovisions of the Agreement.
3. Consultant accepts all terms and requirements of this Addendum by signing
below.
IN WITNESS WHEREOF,the Parties have voluntarily executed this Addendum which will
became effective on the last date written below.
