CONTRACTOR: Totem Electric of Tacoma, Inc.
SUMMARY OF PROPOSED CHANGE:
This change order is a result of the change in grade to the entrance to the parking lot
and regrading the parking lot. Due to the flat grade it was determined that a CB
was needed to prevent ponding.
This work includes connection to existing CB, Type 1 CB compete with frame & grate,
40 LF of 8" PVC drain pipe and CSTC.

subcontractors letter_ The value of this work is set at $3,264.63 as broken down on the attached
worksheet.
It is my understanding it is imperative this work proceed immediately to avoid any further delays
in the construction schedule and to that end we anticipate your prompt approval of this change_
