The payment specified and agreed to in this change order includes every claim by the contractor for any
extra payment with respect to the work described in this change order. The work covered by this change
order must be performed under the same terms and conditions as that included in the original
agreement. All of the other terms of the original agreement remain in effect.

SUBJ: Change Order Request/Acid Stain Restroom Floors
Dear Todd,
Per your request in E-Mail dated January 8,2018 at 1:55PM,pleased to provide lump sum pricing to
complete the Acid Staining the restroom floors. This cost includes providing a Test Sample/Mock-up in
adjacent pump room or storage room. Includes standard surface prep and cleaning. Our previously
submitted extra work price for this additional work of$5,604.92 as shown on our proposal dated 2/5/18 is
now reduced by$2K as shown below for the credit associated with the mow curb discrepancy.Lump sum
price is as follows:
TOTAL LUMP SUM PRICE $ 3,604.92
• We request an additional 2-Contract Day to procure materials &complete this
ADDED scope of work.
This price EXCLUDES Washington State Sales Tax,existing concrete surfacing sand-blasting,grinding
or any other extensive preparation.
Total costs to City of Renton for completing this ADDED Scope of Work is$3,604.92. Please issue us a
Change Order so that we can coordinate this work with Sustainable Interiors. Should you have any
