SECOND ADDENDUM TO OPTION AND SITE LEASE AGREEMENT
The attached Option and Sit Lease Agreement made and entered into the �day of J�nQ A
1991,by and between �� OT ,
("Lessor")and USWEST Communications Wireless Group,a division of USWEST Communications,Inc.,
a Colorado Corporation,("Lessee")of which this Addendum is made a part,is hereby amended and
supplemented as follows:
1.As additional consideration for the removal of First Addendum Paragraph 4,amending
Paragraph 5 of the Option and Site Lease Agreement,subsection(c)referring to the PCS handsets,Lessee
shall pay to Lessor the additional sum of one thousand five hundred dollars($1,500.00).
2. The following shall replace Section 4(a)of the First Addendum to Option and Site Lease
Agreement,modifying Paragraph 5 of the Option and Site Lease Agreement,entitled"Rent:"
(a) Lessee shall pay Lessor,as rent,the sum of$617.40 per month. Beginning in September of
each year,said rent shall be payable in advance to Lessor on or before the first day of each calendar month.
At the beginning of September thereafter,the monthly rent shall automatically be increased in proportion to
the percentage increase in the"All Items"category of the Consumer Price Index for Urban Wage Earners
and Clerical Workers published by the Bureau of Labor Statistics of the US Department of Labor for Seattle
Metropolitan Area(1967+ 100)(the"Index")for the twelve month period ending in July of each year. If an
agreement is executed within six months prior to September,the increases will begin the following
September. If the Consumer Price Index declines during a July to July period,in no event shall the rent be
less than the original rent set forth in the agreement. If the above-referenced Index is discontinued,the
parties agree to use a locally available comparable substitute.
In witness whereof,the parties hereto have executed this instrument by proper persons thereunto duly
authorized so to do on the day and year first herein above written.
