SUMMARY OF PROPOSED CHANGE: Remove and Reset Stop Sign FA.No.20 $239.83 The original approved construction plans and contract provisions did not describe the need for a breakable feature for the removal and resetting of an existing sign. The sign was located at the northeast corner of Harrington Ave NE and NE 8th St.The work was performed on May 2,2018,where the contractor re-removed and reset the stop sign including breaking the paved area near the sign pole. Install Concrete Block Wall FA.No.21 $1,493.15 The original approved construction plans show the rockery wall to be installed approximately 105 LF from Sta.145+60 RT to 146+65 RT.Concern for public safety determined the need for an additional wall installation to fill in the wall gap between the property line of 2805 and 2804 Harrington Ave NE.The work, performed July 13,2018, included placing a concrete block approximately 7 FT long,2.5 FT high at the south end of the rockery wall, hand digging the surface, and back filling with appropriate rock. Cut Pedestrian Curb FA.No.22 $490.31 A new pedestrian curb was installed on April 11,2018.The edge of this new pedestrian curb, located at the north entrance of Breezy Apartments at 802 Harrington Ave NE,was run over by vehicles a couple weeks after installation. Installation of this new curb was deemed necessary for public safety. The work, performed July 13,2018,included cutting the edge of this new pedestrian curb at 45 degree and sack mix the ends. Install a Solid Lid FA.No.23 $453.12 The original approved construction plans did not show the installation of a solid catch basin of the existing CB 2-19.This was determined necessary due to the fact that the catch basin was located on the walk way next to the new concrete sidewalk and did not collect any stormwater runoff. The work was performed on July 16,2018. Asphalt Pavement FA.No.24 $25,592.11 Additional asphalt pavement was needed near Harrington Ave NE&NE 7th St and NE 8`h PI intersection to solve a stormwater runoff collection problem from adjacent streets. The work was as follows: • Harrington Ave NE&NE 7th St intersection-profile milled down from above the new curb to just past the CB 2-21 and repaved to match the existing cross slope and to create slope to curb. • Harrington Ave NE&NE 8th PI-the asphalt at the north and south of NE 8th PI,-65 LF from the existing new stop bar,was profile milled down. The area was repaved to create more crown and invert, cross slope to be minimum 2%for both north and south of NE 8th Pl.A small, depressed area at the north of NE 8th PI&Harrington Ave NE was also milled down to create more slope, where the runoff from the north of Harrington Ave NE&NE 8th PI can flow southerly to a new bio-retention facility. This asphalt work was considered beyond the original scope of work for this project. The work was performed by Lakeside Industries,DPK Inc.'s subcontractor on July 20,2018 H:\File Sys\SWP-Surface Water Projects\SWP-27-Surface Water Projects(CIP)\27-3731 Harrington Ave-Ph_II\1600 Construction\1606 Change Orders\Change Order\CO-2\CO-2.docx\KSmc TOTAL CHANGE ORDER COST: $28,268.52 Additional Working Days added to the Contract for this Change Order: 1 ORIGINAL CONTRACT AMOUNT CURRENT CONTRACT AMOUNT ESTIMATED NET CHANGE ESTIMATED CONTRACT THIS ORDER TOTAL AFTER CHANGE $904,036.00 $925,888.82 $28,268.52 $954,157.34 ..r SIGNATURES; 4),7?1,54 /41,,,,,/6:'-t........
