Description of Work
Construction support to assist in ensuring compliance with environmental requirements for the project.
The Local Agency of City of Renton
desires to supplement the agreement entered in to with DGK,Inc.(dba Widener&Associates)
and executed on 10/28/2015 and identified as Agreement No. CAG-15-209
All provisions in the basic agreement remain in effect except as expressly modified by this supplement.
The changes to the agreement are described as follows:
Section 1, SCOPE OF WORK, is hereby changed to read:
The Scope of Work of the Original Agreement,Supplemental Agreement No. 1 and Supplement Agreement No. 2 is supplemented
to include the attached Exhibit A-3.

Section IV, TIME FOR BEGINNING AND COMPLETION, is amended to change the number of calendar days
for completion of the work to read: The completion date remains 12/31/2018.

Section V, PAYMENT, shall be amended as follows:
See attached Exhibit"A"and Exhibit D-3.The maximum amount payable is increased by$16,109.60 from$20,185.20 to
$36,294.80.
as set forth in the attached Exhibit A, and by this reference made a part of this supplement.

Project: N 27th Place Bridge Scour Repair
PROJECT UNDERSTANDING
Currently the City of Renton has undertaken the construction of the N. 27th Place Bridge Scour Repair
project. The project will repair the scour damage that has occurred to the open bottom concrete arch
culvert. High stream flows have undermined the foundation and thus created the potential for
permanent damage to the structure.
The scope of this work provides for the construction support to assist in ensuring compliance with the
environmental requirements for the project.
Task 2—Construction Support:
• Complete onsite inspections.
• Attend weekly construction meetings.
• Making recommendations for environmental compliance.
• Assist with the planting plan.
Deliverables
• Notes and email recommendations.
