HomeMy WebLinkAboutAdden 1 CAG-18-279,Adden#1-19 gib ws AMENDMENT NO. 1 TO AGREEMENT FOR EVALUATION SERVICES FOR RENTON SENIOR ACTIVITY CENTER THIS AMENDMENT, dated January 24, 2019, is by and between the City of Renton (the "City"), a Washington municipal corporation, and Margaret McKenna ("Consultant"), of ConTEXT Evaluation, an individual/sole proprietor. The City and the Consultant are referred to collectively in this Amendment as the "Parties." Once fully executed by the Parties, this Amendment is effective as of the last date signed by both parties. Whereas, the City engaged the services of the Consultant under Agreement CAG-18-279, dated December 12, 2018,to provide necessary services for the Evaluation Services for Renton Senior Activity Center (referred to herein as the "Agreement"); Whereas, the Parties wish to amend the Agreement to extend it through March 31, 2019, and add $3,500 in payment, in order to gather and analyze additional data and feedback from diverse older adults in the greater Renton area. NOW THEREFORE, It is mutually agreed upon that CAG-18-279 is amended as follows: 1. Scope of Work: Section 1, Scope of Work, is amended in order to gather and analyze additional data and feedback from diverse older adults in the greater Renton area as specified in Exhibit A-1, which is attached and incorporated herein. 2. Time of Performance: Section 3, Time of Performance, is amended pursuant to the schedule(s) set forth in Exhibit A-1. All Work shall be performed by no later than March 31, 2019. 3. Compensation: Section 4, Compensation, is amended so that the maximum amount of compensation payable to Consultant during the January to March 2019 time period is $3,500 plus any applicable state and local sales taxes, bringing the Agreement's total compensation to $7,500, plus any applicable state and local sales taxes. The additional compensation shall be paid based upon Work actual performed according to the rate(s) or amounts specified in Exhibit A-1 of the Agreement. 4. All terms of the Agreement not explicitly modified herein shall remain in full force and effect and such terms shall apply to Work performed according to this Amendment as if fully set forth herein. • IN WITNESS WHEREOF, the Parties have voluntarily entered into this Agreement as of the date last signed by the Parties below. In our first few months of the Senior Center Needs Assessment (fall 2018), we surveyed over 400 older adults, but were lacking in feedback from African American, Hispanic and Vietnamese seniors. For the time period of January — March 2019, Ms. McKenna will develop assessment tools, hold focus groups, and analyze data from these three groups, to provide more comprehensive feedback and information for future Renton Senior Activity Center programming and planning. This project is funded by King County's Veterans, Seniors and Human Services Levy funding. Contract Deliverables: • Attend and participate in grant related project trainings and meetings with Renton Senior Activity Center staff and advisory boards as needed. • Develop data collection tools including surveys, interview guides, and focus group guides for African American, Hispanic and Vietnamese older adults. • Train Senior Center"Ambassadors" on how to administer surveys and collect participant data. • Gather new survey, key informant,and focus group findings and conduct data analysis, and incorporate with previous findings. • Develop a comprehensive summary report and present findings to interested committees, boards,community members and staff by end of March 2019.
