SUMMARY OF PROPOSED CHANGE:
Provide all labor,equipment,subcontracting, bonding, insurance for the complete work required for the extra
work as outlined below and shown in the contractor's proposal:

Pulverize Existing Road Pavement-This item shall include pulverizing the existing road
pavement,grading and compacting the pulverized asphalt to form a sub-base for new
HMA pavement,removal,haul and disposal of excess pulverized asphalt.

Hot Mix Asphalt Paving-This item shall include Hot Mix Asphalt(per specification 5-
04 of the project specifications),3.5"in depth installed in one pass,incidental paving
items shall be per specification 1-09.14(2)H.

Asphalt Grinding-This item shall include grinding 2"of existing asphalt road,removal,
haul and disposal of asphalt grindings.
