The revised grading plan did not result in a change in Bid Item quantities. • Delete the following Bid Items from the Contract: o FURNISHING STEEL SHEET PILING, Quantity= 20 Ton o INSTALL STEEL SHEET PILING, Quantity= 100 L.F. • Add the following Bid Items to the Contract: o FURNISHING STEEL SHEET PILING, Quantity= 10.06 Ton o INSTALL STEEL SHEET PILING, Quantity=44 L.F. o INSTALL CONCRETE CUTOFF WALL,Quantity= 1 L.S. • Add Working Day(s)to the contract for the additional time required to excavate, install and strip concrete forms, install and tie reinforcing steel and place concrete for the Concrete Cutoff Wall. Refer to the Extension of Time section of this change order for the number of Working Days added to the contract. • Add Special Provision Section 6-02.2. 6-02.2 Materials Section 6-02.2 is supplemented with the following: Epoxy Resin 9-26.1 The Epoxy Resin Adhesive used to anchor the reinforcing steel of the Concrete Cutoff Wall into existing culvert concrete footings shall meet ASTM C881 Type IV, Grade 3, Class B and C. • Special Provision Section 6-02.4 is supplemented with the following: Concrete Cutoff Wall shall be measured by lump sum. Change Order No. 5 Contract Title: N 27th Place Bridge Sour Repair Project Contract No. CAG-18-002 Page 2 of 5 • Add Special Provision Section 6-02.5. 6-02.5 Payment Section 6-02.5 is supplemented with the following: "Concrete Cutoff Wall", per lump sum, shall include all labor, equipment and materials to construct the concrete cutoff wall as described in the plans and specifications. The Contractor shall receive 100 percent of the lump sum Contract price upon completion of the concrete cutoff wall. Reason for Change: Upon completion of the installation of the sheet piles for wall 1, pile driving started on wall 2. Very stiff soil conditions were encountered in the alignment of wall 2 that prevented the installation of the sheet piles using a vibratory hammer. Several alternatives were considered by the Construction Team (City, Contractor, Construction Management Team and Design Team) from the use of a larger crane and diesel pile driving hammer to install the sheet piles,to excavating through the stiff clay layer in order to install the sheet piles,to the deletion of the sheet pile wall 2 and replacing it with a concrete cutoff wall installed at the outlet end of the existing culvert. The stiff clay layer encountered was unexpected resulting in a changed condition.The thickness of the stiff clay layer was unknown. Demobilizing the existing crane and vibratory hammer and mobilizing a larger crane and larger diesel pile driver is expensive and time consuming.The steep slopes of the deep narrow ravine limit the depth of any additional excavation needed to get through the stiff clay layer.Additional excavation in the immediate vicinity of the creek increases the risk of introducing turbidity into the creek water. Given all these factors, it was determined by the Construction Team that the deletion of the sheet pile wall and installation of a concrete cutoff wall posed the least risk for the project in terms of potential delay, added expense and environmental impacts. As a result of this change, the adjusted final quantity of the bid item FURNISHING STEEL SHEET PILING is 50%of the original Bid quantity and the adjusted final quantity of the bid item INSTALL STEEL SHEET PILING is 44%of the original Bid quantity. Pursuant to 1-04.6,the Contractor is entitled to an equitable adjustment since the adjusted final quantity is less than 75%of the original Bid quantity. Change Order No. 5 Contract Title: N 27th Place Bridge Sour Repair Project Contract No. CAG-18-002 Page 3 of 5 Materials: • Concrete Cutoff Wall o Concrete Class 4000 o Reinforcing Steel o Epoxy Resin Adhesive: ASTM C881, two-component,Type IV, Grade 3 Class B and C Plans: Refer to drawings attached to this Change Order. Adjustment: This Change Order includes a significant change as defined in Section 1-04.4. Specifically,the deletion of work resulting in decreased quantities and the addition of work with new bid items. The estimated cumulative total cost of this change is$7,500. The estimate is attached. Extension of Time: The Time for Completion is extended 3 non-compensable Working Days in accordance with Section 1-08.8. Equitable Adjustment: This Change Order includes an equitable adjustment in the new unit price for the steel sheet pile bid items and the new concrete cutoff wall bid item. Change Order No. 5 Contract Title: N 27th Place Bridge Sour Repair Project Contract No. CAG-18-002 Page 4 of 5 Sections 1-04.4 and 1-04.5 of the 2016 WSDOT/APWA Standard Specifications shall govern this Change Order.The Work of the referenced Contract is modified to include the changes detailed herein.The payment provided for herein shall constitute the complete and final settlement for all costs of labor, equipment, materials, overhead, profit, permit fees, and all other claims that may be made by the Contractor as a result of this change.
