Dear Nate,
Please use this change request form as an Addendum to our Agreement.This Addendum provides for
the following additional changes for Lockbox services and corresponding fees as listed under
Attachment A of this document.
• Current'Payment Frequency' indexing field options are:
o A-Annual
o Q-Quarter
o M-Monthly
• Retail Lockbox, Inc.will add the following indexing field option for Payment Frequency:
o R-Renewal
o "R" will be used for Business Annual License
o If Payment Frequency ="R":
■ Image metadata file/filename—Use"Renewal"as Document Type
• Posting File XML—Change Payment Frequency to"A"
• RetaiIWEB'" Upload—Change "PaymtFreq"to "A"
• For Business License Renewal Zero-Pay items, if Correspondence Flag= "Y"and Code 88=$0,
Retail Lockbox, Inc.will change Document Type in Image metadata file/filename from "Renewal"
to"Correspondence".
The terms defined in the original Agreement,which are used herein, shall have the same meaning and
duration except as stipulated below.
