CALL TO ORDER:
The May meeting was called to order at 8:00 a.m.by T.Nelson.
APPROVAL OF MINUTES:
Motion made by J. Phelan, Seconded by D. Law and Carried to approve the minutes for the
month of April 24,2007 Regular Meeting.
CORRESPONDENCE:
The request from Retiree 8845 regarding acupuncture was discussed. It was deternuned the
request did not meet the established criteria for reimbursement. Staff was directed to write to
Retiree 8845 and explain the Board's decision.
iTNFINISHED BUSINESS:
Active member 8146's notification of return to duty from disability leave was reviewed. J.
Phelan moved to return active member 8146 to work effective May 14, 2007. The motion was
seconded by D.Law;the motion carried.
NEW BUSINESS:
A request for disability leave from active member 2783 for a low back injury was reviewed. The
injury was not in the line of duty. The anticipated time frame of the disability is one to three
months. J. Phelan moved to approve the disability leave effective May 23, 2007. The motion
was seconded by R. Anderson; the motion carried.

M. Webby provided an update on the status of the city's review of long term care insurance for
LEOFF 1 members. New York Life and Aflac are interested in providing coverage.
Approximately 55 people have made it through the first step. M. Webby and T. Shuhart have
talked with the actuary and are waiting for the final premium information from the broker. When
the review is complete staff will talk with J. Covington and the City Council to see if they would
like to fund the premiums. At this time it appears the cost to insure would be approximately
$116,000 to$130,000 per year.
Active member 4460's return to duty form will be available at the next LEOFF Disability Board
Meeting.
LEOFF 1 Retiree George Conklin died in April.
APPROVAL OF BII..LS:
The Board Members reviewed the claims processed under the Secretary's authority during April,
2007.
COMMENTS/ANNOUNCEMENTS:
B.Deines will not be able to attend the June Disability Board Meeting.
The next regular meeting of the LEOFF Board is scheduled for June 26, 2007 at 8:00 a.m. in the
HR Conference Room.
ADJOURNMENT:
There being no further business,the meeting was adjourned at 8:45 a.m.
