The meeting was called to order at 8:00 by Jim Phelan.

Member at Large APPROVAL OF MINUTES
Greg Taylor K. Parker moved and R. Anderson seconded approval of the minutes of
Council Member December 15, 2009.
NEW BUSINESS
A motion was made by K. Parker and Seconded by R. Anderson to nominate 1im
Phelan as Chair of the LEOFF Disability Board for 2010. The motion carried.
CORRESPONDENCE
A request was received from Retiree 1431 to continue to receive massage
therapy approximately two times per month indefinitely to treat his lumbar disc
bulges and para spinal spasms, stiffness, and pain. A motion was made by K.
City Staff Parker and seconded by R. Anderson to approve the massage therapy for a
Terri shuhart ninety day period and direct staff to correspond with Retiree 1431 and request a
Secretary report from the retiree's physician to provide a follow-up report and a treatment

A request was received from retiree 1603 for reimbursement for the purchase of
'�._�r?�-��: absorbent pads in the amount of $32.27. A motion was made by R. Anderson
and seconded by 1. Matthew to approve the reimbursement. The motion
carried.

during January 2010.

December 14.
ADJOURNMENT
There being no further business of the Board, R. Anderson moved and K. Parker
seconded the adjournment of the meeting at 8:30.
