To remedy this problem, the City will place longitudinal restrainers at the top and bottom flange of each girder. These restrainers will attach to the girders themselves, extend through the pier caps, and be tightened to a desired tension. Additionally, a shelf (corbel) will be placed at the bottom of each girder in the vicinity of the cracks to act as a fail-safe device. Once all of the bracing is in place, an epoxy grout will be injected into each of the cracks to prevent weathering of the steel within the concrete. All of the required work will be done from either the bridge deck or from suspended scaffolding set up by the contractor. The proposed staging area for the construction equipment has not been determined at this time. Construction should commence in late August and extend through November. Specific Project Narrative Project name, size, and location of site: The purposed project is the Monster Road Bridge Repair. The bridge deck is approximately 8,600 SF in size and is located over the Black River just south of the Stoneway Quarry (Sec 13, T 23, R 4 E, WM). lAnd use permits required for proposed project: Shor~line Exemption, Hydraulic Project Approval Zoning designation of the site and adjacent properties: ill/1M/CO Current use of the site and any existing improvements: The Monster Road Bridge serves as a North/South thoroughfare over the Black River. The repair shall improve the bridges safety and longevity. This will be accomplished through steel bracing attached to each of the girders and applying a post tension force. Special site features (i.e. wetlands, water bodies, steep slopes): The Black River and BNSF are the most prominent features in the area. 1 / Statement addressing soil type and drainage conditions: N/A Proposed use of the property and scope of the proposed development: N/A For plats indicate the proposed number, density and range of sizes of the new lots: N/A Access: Access to the construction site will be from Monster Road. Proposed off-site improvements (i.e. installation of sidewalks, fire hydrants, sewer main, etc.): N/A Total estimated construction cost and e~timatedfair market value of the proposed project: $500,000.00 Estimated quantities and type of materials involved if any fill or excavation is proposed: N/A Number, type and size of any trees to be removed: NI A Explanation of any land to be dedicated to the City: N/A Proposed number, size or range of sizes of the new lots and density (if applicable): NI A Any proposedjob shacks, sales trailers, and/or model homes: Job shack usage is undetermined at this time. There may be one for the contractor located near the bridge, but this is not known at this time. Any proposed modifications being requested: All modifications will be done to the bridge girders. No additional modifications will be done to the structure. Distancefrom closest area of work to the ordinary high water mark of the proposed project site: All of the work done to the bridge will be over the Black River. 2 Nature of the existing shoreline: Most of the shoreline along the Black River (within the project limits) consists of sands and gravels with a variety of low growing vegetation. The riverbed consists of cobbles, gravels, and sands with little streambed vegetation. Quarry spalls (rip rap) provide protection for the bridge abutments beneath the Monster Road Bridge. The approximate location of and number of residential units, existing and potential, that will have an obstructed view in the event the proposed project exceeds a height of 35-feet above the average grade level: NI A 3 Monster Road Bridge Repair Project o 200 400 1 : 2400 Monster Road Bridge R · p.
