Under each guardrail post mounted to the sheet pile cap beams. *** • Add Special Provision Section 6-02.3(25)0 Girder to Girder Connections. 6-02.3(25)0 Girder to Girder Connections The second paragraph is revised to read: Prestressed concrete girders shall be constructed in the following sequence: 1. If required, deflections shall be equalized in accordance with the Contractor's equalization plan. 2. Any intermediate diaphragms shall be placed and any weld ties shall be welded in accordance with Section 6-03.3(25). Welding ground shall be attached directly to the steel plates being welded when welding the weld-ties. 3. Any keyways between adjacent girders shown in the Plans to receive grout shall be filled flush with the surrounding surfaces using a grout mortar conforming to Section 9 20.3(2}9-20.4(4). 4. Equalization equipment shall not be removed and other construction equipment shall not be placed on the structure until the intermediate diaphragms and keyway grout have attained a minimum compressive strength of 2,500 psi. Reason for Change: Pursuant to contract CAG-18-001, grout was to be used for the grout pads at abutments 1 and 2, girder shear keys and under the thrie beam post bearing plates located on the sheet pile caps. Below is a list of bid items that included grout along with the type of grout specified for each application. Change Order No. 8 Contract Title: NE 31st St Bridge Replacement Project Contract No. CAG-18-001 Page 2 of 6 • Bid Item#35 Prestressed Conc.Girder—30"Voided Slab—CAG-18-001,Volume 1,Special Provision Section 6-02.5, Payment, states "The fourteenth bid item "Prestressed Conc. Girder ", under Section 6-02.5 is supplemented with the following: The contract unit price per linear foot of Precast Conc. Girder—30"Voided Slab includes all the Work described in Sections 6-02.3(25)and 6-02.3(25)A through 6-02.3(25)0." CAG-18-001, Volume 1, Amendments, Section 6-02.3(25)0, Girder to Girder Connections,item 3 states"Any keyways between adjacent girders shown in the Plans to receive grout shall be filled with the surrounding surfaces using grout conforming to Section 9-20.3(2)." Grout for the girder shear keys was included in this bid item. The 2016 WSDOT Standard Specifications, Section 9-02.3(2) Grout Type 2 for Nonshrink Applications states "Grout Type 2 shall be a nonshrink, prepackaged material meeting the requirements of ASTM C1107. The minimum compressive strength shall be 4,000 psi at 7 days." • Bid Item #36 Superstructure— NE 31'Street Bridge—CAG-18-001, Volume 1, Special Provision 6- 02.5, Payment, states "The third bid item under Section 6-02.5 is supplemented with the following: Bridge and Structures Minor Items—For the purpose of payment,such bridge and structures items as ***grout pads, pre-molded joint filler, rubber joint sealer, falsework, formwork, hardware ***etc., for which there is no pay item included in the proposal, are considered as bridge and structures minor items.All costs in connection with furnishing and installing these bridge and structures minor items as shown and noted in the Plans and as outlined in these specifications and in the Standard Specifications shall be included in the ***Superstructure—NE 31st Street Bridge. ***"Grout for the grout pads was included in this bid item. CAG-18-001, Volume 1 Special Provision Section 6-02.3(20) Grout for Anchor Bolts and Bridge Bearings,states "Section 6-02.3(20)is supplemented with the following:Grout placed at the following locations shall conform to the requirements of this section. ***All grout pad locations for bearing pads. Under each guardrail post mounted to the sheet pile cap beams. *** " The 2016 WSDOT Standard Specification Section 6-02.3(20), Grout for Anchor Bolt and Bridge Bearings, states "Grout shall conform to Section 9-20.3(2) for anchor bolts and for bearing assemblies with bearing plates. Grout shall conform to Section 9-20.3(3)for elastomeric bearing pads and fabric pad bearings without bearing plates." The 2016 WSDOT Standard Specifications, Section 9-02.3(3) Grout Type 3 for Unconfined Bearing Pad Applications states "Grout Type 3 shall be a prepackaged material meeting the requirements of ASTM C928, Table 1, R2 Concrete or Mortar." • Bid Item#64 Rail on Sheet Pile Cap Beam—CAG-18-001,Volume 1,Special Provision 8-11.5,Payment, states "Section 8-11.5 is supplemented with the following: "Rail on Sheet Pile Cap Beam, per linear foot. The unit price for "Rail on Sheet Pile Cap Beam"shall be full payment for all costs to furnish and install the rail, including posts, anchors, base plates, hardware, etc. as shown on the plans." Grout placed under the thrie beam bearing plates located on the sheet pile caps is included in this bid item. CAG-18-001, Volume 1 Special Provision Section 6-02.3(20) Grout for Anchor Bolts and Bridge Bearings,states "Section 6-02.3(20)is supplemented with the following:Grout placed at the following locations shall conform to the requirements of this section. ***All grout pad locations for bearing pads. Under each guardrail post mounted to the sheet pile cap beams. *** " The 2016 WSDOT Standard Specification Section 6-02.3(20), Grout for Anchor Bolt and Bridge Bearings, states "Grout shall conform to Section 9-20.3(2) for anchor bolts and for bearing assemblies with bearing plates. The minimum compressive strength shall be 4,000 psi at 7 days." Submittal No. 29 included a Request for Approval of Material (RAM) for Target Products Ltd. Portland Expanding (Non-Shrink) Grout. The Contractor sent the submittal via e-mail on June 8, 2018 to the Construction Management Consultant for review. The submittal was returned to the Contractor on September 12,2018.The RAM listed Bid Item No.35 Prestressed Conc.Girder-30"Voided Slab,Bid Item No. 36 Superstructure—NE 315t Street Bridge, Bid Item No. 64 Rail on Sheet Pile Cap Beam. A product sheet was included with the RAM. Per the remarks on the Transmittal cover,the grout was to be utilized under the guardrail mounted on the sheet pile caps, grout under the bearing pads, and the grout fill of the girder keyways.The grout fill in the girders was to be extended utilizing the pea gravel included in the RAM. The response on the Transmittal cover was "Approved as Noted — See RAM". Construction Management Consultant Reviewer No. 1 coded 1 and 8 on the RAM for the acceptance criteria. Code 1 states"Acceptance based upon"Satisfactory"Test Report for samples of materials to be incorporated into project." Code 8 states "Source Approved". This product is listed on the WSDOT QPL which references Standard Specification 9-20.3(2), Grout Type 2 for Nonshrink Applications, and Standard Specification 9- 20.3(4),Grout Type 4 for Multipurpose Applications.A WSDOT QPL sheet was not included with Submittal No. 29. Submittal No.39 included a RAM for Target Products Ltd.Traffic Patch.The Contractor sent the submittal via e-mail on August 23,2018 to the Construction Management Consultant for review.The submittal was returned to the Contractor on August 24, 2018. The RAM listed Bid Item No. 36 Superstructure—NE 315t Street Bridge.A product sheet was included with the RAM. Per the remarks on the Transmittal cover,the grout was to be used for girder keyways and grout pads.The response on the Transmittal cover was"No Exceptions Taken". Construction Management Consultant Reviewer No. 2 coded 1 on the RAM. Code 1 states"Acceptance based upon "Satisfactory"Test Report for samples of materials to be incorporated into project" This product is listed on the WSDOT QPL which references Standard Spec. 9-20.2, Concrete— Concrete Patching and Dowel Bar Retrofit, and Standard Spec. 9-20.4(4), Mortar Type 3 for Concrete Repair.A WSDOT QPL sheet was not included with Submittal No.39. Target Traffic Patch Fine(Rapid Setting,Fibre Reinforced High Early Strength,Mortar Repair Mix)was used for the grout pads at abutments 1 and 2 (Sept. 13, 2018), shear keys between girders (Oct. 1, 2018 and Oct. 2,2018) under each guardrail post mounted on the sheet pile cap beams (Oct. 23, 2018 and Oct. 24, 2018). Although Submittal No. 39 was submitted after Submittal No. 29, Submittal No. 39 was approved and returned to the Contractor on August 24, 2018 before Submittal No. 29 was approved and returned to the Contractor on September 12,2018.Submittal No.29 was returned one day prior to the pouring of the grout pads on September 13, 2018. The material approved for Submittal No. 39 was purchased and delivered to the site in preparation for the pouring of grout pads. The review and approval of both submittals were performed by different Construction Management Consultant Reviewers.The approvals of both submittals resulted in a discrepancy in the type of material approved for use. Additionally, both approvals were inconsistent with the materials specified in the contract documents. Change Order No. 8 Contract Title: NE 31st St Bridge Replacement Project Contract No. CAG-18-001 Page 4 of 6 There is no evidence in the project construction files that the Engineer of Record reviewed or approved these submittals prior to their return to the Contractor. When the discrepancy was discovered, the Engineer of Record was asked if the material used was acceptable for each application. In an e-mail dated November 9,2018,the Construction Manager responded to the City Project Manager's inquiries regarding the use of the Target Traffic Patch for those locations that were designated to have non-shrink grout per the contract.The Engineer of Record was included on the distribution list in the response.The response stated that the City's questions were discussed with the Engineer of Record and the Manufacturer. The conclusion was that the Target Traffic Patch meets the design requirements despite not being classified as a non-shrink grout. The response asserted that the Target Traffic Patch sets so rapidly there is no shrinkage or net volume change. The Target Traffic Patch Fine has essentially the same compressive strength as Target Portland Expanding Grout(non-shrink).According to the product data sheet from the manufacturer,the compressive strength (ASTM C109) is typically 7,500 psi at 28 days for Traffic Patch Fine and 7,820 psi at 28 days (flowable mix consistency)for the non-shrink grout. However,the bond strength for Target Traffic Patch Fine is superior to Target Portland Expanding Grout (non-shrink). According to the product data sheet from the manufacturer,the bond strength(ASTM C882)is typically 1,890 psi at 7 days for Traffic Patch Fine whereas a typical bond strength is not listed for the non-shrink grout. Materials: Mortar Type 3 for Concrete Repair Plans: No changes were made to the drawings. Adjustment: This Change Order includes changes as defined in Section 1-04.4. Specifically, altering specifications. Extension of Time: The Time for Completion is extended 0 non-compensable Working Days in accordance with Section 1-08.8. Equitable Adjustment: This Change Order does not include an equitable adjustment by agreement with the Contractor. Change Order No. 8 • Contract Title: NE 31st St Bridge Replacement Project Contract No. CAG-18-001 _ Page 5 of 6 Sections 1-04.4 and 1-04.5 of the 2016 WSDOT/APWA Standard Specifications shall govern this Change Order.The Work of the referenced Contract is modified to include the changes detailed herein.The payment provided for herein shall constitute the complete and final settlement for all costs of labor,equipment, materials,overhead, profit, permit fees,and all other claims that may be made by the Contractor as a result of this change.
