SUMMARY OF PROPOSED CHANGE:
Extra Grout Concrete Mix $18,091.14
During project construction,a significant additional amount of grout concrete mix(1,266
gallons)was needed to repair the bottom of the existing 36"CMP Culvert prior to the
installation of the new Cure-In Place Pipe(CIPP).This was due to the existing CMP
culvert being in more deteriorated conditions than anticipated and the inability to •
completely CCTV prior to construction phase.The payment will be made in accordance
with the contractor's unit bid price.The work was performed on October 11,2019.
TOTAL CHANGE ORDER COST: $18,091.14
