For each Small Cell, the Company shall fill out a Site License Addendum. This Site License Addendum (“Addendum”), made this _____ day of ____________, 2021 (the “Site License Addendum Effective Date”) between the City of Renton, hereinafter designated the “City” and New Cingular Wireless PCS, LLC, with its principal offices at 1025 Lenox Park Boulevard NE, 3rd Floor, Atlanta, GA 30319, hereinafter designated “Company”: 1. Addendum. This is a Site License Addendum as referenced in that certain Master License Agreement between The City and Company dated December 15, 2020 ("Agreement"). All of the terms and conditions of the Agreement are incorporated hereby by reference and made a part hereof without the necessity of repeating or attaching the Agreement. In the event of a contradiction, modification or inconsistency between the terms of the Agreement and this Addendum, the terms of this Addendum shall govern. Capitalized terms used in this Addendum shall have the same meaning described for them in the Agreement unless otherwise indicated herein. 2. Project Location. Attachment 1 identifies the physical location of the City Poles on which the Site Equipment shall be attached. 3. Project Description. Attachment 2 identifies the Site Equipment to be installed on the City Poles and shall include the construction details (e.g. electrical and fiber connections, antennas, radios and accessory components). If deviating from the standard detail, Company shall provide photo simulations of such Site Equipment attached to the City Poles and a load bearing study. The photo simulations shall include photos indicating the existing City Pole without the Site Equipment and with the proposed Site Equipment attached. If there are any accessory components, for example conduit holding backhaul or electrical, such accessory components shall be depicted in the photo simulations. 4. NIER Report. Company has commissioned an NIER Report for the City Poles. Such report is attached hereto in Attachment 3. 5. Backhaul. Backhaul services shall be provided to the Site Equipment as follows: □ underground fiber □ aboveground fiber □ microwave □ other_________________ and from the following entity: __________TBD__________________. Access to the backhaul system shall be separate from the City’s communication and electrical conveyance system. The Company, or its third party contractor, is responsible for the placement and installation of any junction boxes. 6. Electrical Fee. The estimated peak draw power for the Small Cell is _15 kWh__. The Small Cell electrical usage falls within City cost tier as described in the Fee Schedule. 7. Usage of City-Owned Conduit. A radio equipment enclosure will be installed on the same pole. The proposed facility will comply with the prevailing standards for public exposure limit on RF energy. The conclusion is based on information provided by Mastec/AT&T, on site measurement of existing RF field strength and the RF power density calculations performed in this study. Members of the General Public will not be exposed to hazardous levels of RF energy in the surrounding area of the facility. No. 37962 May 30, 2020 PROTOCOL This study, and the calculations performed herein, is based on OET Bulletin 651 which adopts ANSI C95.1- 1992 and NCRP standards. In particular, equation 10 from section 2 of the guideline is used as a model (in conjunction with known antenna radiation patterns) for calculating the power density at different points of interest. This information will be used to judge the RF exposure level incident upon the general population, and any employee present in the area. It should be noted that ground reflection of RF waves has been taken into account. FCC Maximum Permissible Exposure Levels The FCC’s maximum permissible exposure (MPE) is defined as “the rms and peak electric and magnetic field strength, their squares, or the plane-wave equivalent power densities associated with these fields to which a person may be exposed without harmful effect and with an acceptable safety factor level”. The FCC defines two exposure environments based on the awareness of persons who are being exposed. RF safety compliance centers on management of these two environments. 1 Cleveland, Robert F, et al. Phone: 1 (778) 805-2166 www.coreoneconsulting.com General Population/Uncontrolled Exposure The General Population/Uncontrolled exposure applies to human exposure to RF fields when the general public is exposed or in which persons who are exposed as a consequence of their employment may not be made fully aware of the potential for exposure or cannot exercise control over their exposure. Therefore, members of the general public always fall under this category when exposure is not employment-related. Occupational/Controlled Exposure The Occupational/Controlled exposure applies to human exposure to RF fields when persons are exposed as a consequence of their employment and in which those persons who are exposed have been made fully aware of the potential for exposure and can exercise control over their exposure. These exposure limits also apply where exposure is of a transient nature as a result of incidental passage through a location where exposure levels may be above General Population/Uncontrolled limits as long as the exposed person has been made fully aware of the potential for exposure and can exercise control over his or her exposure by leaving the area or by some other appropriate action. The FCC’s MPE limits for the General Population/Uncontrolled and Occupational/ Controlled environments are given in Table 1 and Table 2, respectively. Figure 1 is a graph of both MPEs as functions of frequency. Phone: 1 (778) 805-2166 www.coreoneconsulting.com In general, as specified in 47 C.F.R. 1.1307(b), as amended, when the FCC's guidelines are exceeded in an accessible area due to the emissions from multiple fixed transmitters the following policy applies. Actions necessary to bring the area into compliance with the guidelines are the shared responsibility of all licensees whose transmitters contribution to the RF environment at the non-complying accessible area exceeds 5% of the exposure limit (that applies to their particular transmitter) in terms of power density or the square of the electric or magnetic field strength. This applies regardless of whether such transmitters would, by themselves, normally be excluded from performing a routine environmental evaluation. Owners of transmitter sites are expected to allow applicants and licensees to take reasonable steps to comply with the FCC's requirements and, where feasible, should encourage collocation of transmitters and common solutions for controlling access to areas where the RF exposure limits might be exceeded. RF Safety Signs AT&T uses two types of RF safety signs: • Alerting signs • Information signs Alerting Signs Three types of alerting signs, NOTICE, CAUTION, and WARNING, associated with increasing exposure risk relative to the applicable MPE limits are used. They announce the existence of potential RF exposure hazards within the areas they demarcate. CAUTION signs alert persons that they are attempting to enter an area in which RF exposure may exceed the Occupational/Controlled MPE. WARNING signs alert persons that they are attempting to enter an area in which the RF exposure may exceed the Occupational/Controlled MPE by a factor of 10 or greater The NOTICE sign should be installed on the post 4 feet below the antenna. Information Signs INFORMATION signs instruct persons on what to do and who to contact if they are in, or desire to enter, an area where AT&T operates antennas. Phone: 1 (778) 805-2166 www.coreoneconsulting.com Information Sign 2 Antennas that are stand-alone (e.g., lamp posts, stadiums) in areas where there is little potential for exceeding the General Population/Uncontrolled MPE except, perhaps, very close to the antenna. In this case, the INFORMATION Sign 2 would be mounted close to the antenna in order to alert maintenance workers. (Note: In areas where the potential exists for exceeding the GP MPE limits, additional compliance action, including alerting signs, may be needed.). MITIGATION MEASURES: Due to the mounting height of the proposed AT&T antenna, it would not be accessible to the general public. Phone: 1 (778) 805-2166 www.coreoneconsulting.com ON SITE MEASUREMENTS The site was visited on May 28, 2020 at 12:06 pm. The weather at the time of the visit was a cloudy afternoon with a temperature of 74°F. All accessible areas of the site were inspected. Measurement collection was performed using Narda Survey Meter and Broadband Probe (300kHz to 50 GHz) and was consistent with FCC and Narda procedures in making slow sweeping motions over the area that a person could occupy. In using this broadband instrument, the results represent the cumulative contributions of all RF sources at the measurement locations. Narda Radiation meter NBM-550-S / F-0266 (Calibration Date: June 21, 2018) and probe EA-5091 (NBM) (Calibration Date: June 21, 2018) were used to record spatially averaged power density levels as a percentage of the FCC MPE General Population MPE limits. Table 3 below is the collection of measurement readings. Table 3: Measurement Readings are Spatial Average and Maximum as MPE % of the FCC General Population Limits Measurements taken during the site visit attest that the current maximum spatial average power density reading is 0.67% of the MPE limit for general population/uncontrolled exposure in the surrounding area of the proposed facility. There are no locations in the surrounding area near the facility that will have RF exposure levels close to the MPE limit. In order to evaluate the RF exposure level, the power densities at different locations of interest have been examined.
