HomeMy WebLinkAboutAdden 1AMENDMENT NO. _1_ TO 2021-2022 HUMAN SERVICES AGREEMENT CAG# 21-073 THIS AMENDMENT, dated for reference purposes only as May 4, 2022, is by and between the City of Renton (the “City”), a Washington municipal corporation, and Way Back Inn Foundation, a Washington nonprofit corporation (“Agency”). The City and the Agency are referred to collectively in this Amendment as the “Parties.” Once fully executed by the Parties, this Amendment is effective as of the last date signed by both parties. Whereas, the City engaged the services of the Agency under Agreement CAG-21-073, dated March 30, 2021 to provide necessary services for the Transitional Housing Program (referred to herein as the “Agreement”); Whereas, the Parties wish to amend the Agreement to increase the compensation in order to serve additional Renton residents. NOW THEREFORE, It is mutually agreed upon that CAG-21-073 is amended as follows: 1. Scope of Work: Section 1, Scope of Work, in Exhibit A is amended to increase the scope of work for 2022 as described below: 1st Qtr 2022 2nd Qtr 2022 3rd Qtr 2022 4th Qtr 2022 2022 Totals Estimated 2022 Qtrly amount $3,448 $6,450 $6,451 $6,451 $22,800 Unduplicated City of Renton clients served 6 9 9 9 33 Number of transitional housing bednights 259 350 350 350 1,309 Number of Case management meetings 15 20 20 20 75 2. Compensation: Section 4, Compensation, is amended so that the maximum amount of compensation payable to Consultant is increased by $12,800 for 2022 services, from $10,000 to $22,800 for 2022. The additional compensation shall be paid based upon Work actually performed according to the rate(s) or amounts specified below:           CAG-21-073, Adden #1-22 PAGE 2 OF 2 2022 Estimated Budget $22,800 for 2022 Personnel $ 8,000 Non-Personnel (Utilities, rent, repair/maintenance costs of Renton units) $14,800 3. Notices: Section 16, is amended to change the City of Renton contact to: Lori Fleming Human Services Coordinator 1055 South Grady Way, 1st floor Renton, WA 98057 Phone #425-650-6655 Lfleming@rentonwa.gov 4.All terms of the Agreement not explicitly modified herein shall remain in full force and effect and such terms shall apply to Work performed according to this Amendment as if fully set forth herein.
